Coverage Criteria for Opioids in Neuropathic Pain

    Initial Therapy

Covered when the sequential criteria below are met:

    Initial criteria sequence

    • Criterion 1: Request is for a short-acting opioid.

      If not met, deny.

    • Criterion 2: Documentation supports a funded diagnosis of neuropathic pain.

      If not met, deny.

    • Criterion 3: Member has had an adequate trial of (or documented contraindication to) two agents in each of the four recommended non-opioid drug classes (tricyclic antidepressants, SNRIs, gabapentinoids, sodium channel blockers).

      If not met, deny.

    • Criterion 4: If the request is for tramadol, approve for 6 months.

If met, approve for 6 months.

    • Criterion 5: If not tramadol, member must have had an adequate trial of or documented contraindication to tramadol before approving other short-acting opioids (approval up to 50 MME/day for 6 months).≤ 50 MME/day

If not met, request provider to consider trial of tramadol; if met, approve up to 50 MME/day for 6 months.

Trials of multiple agents/classes requirement (supporting detail)

    • Class trial requirement: Adequate trials (or documented contraindications) of two agents in each of the four first-line classes (tricyclic antidepressants, SNRIs, gabapentinoids, sodium channel blockers) have been completed before opioid approval.

      If not met, deny.

    Continuation/Renewal Therapy

Renewal approvals require continuing demonstration of benefit and risk assessment:

    Renewal criteria sequence

    • Renewal 1: Member met all initial criteria.

      If not met, deny renewal.

    • Renewal 2: There is documented reduction in pain and/or improved function.

      If not met, deny renewal.

    • Renewal 3: There is documented opioid risk assessment (eg PDMP verification, ORT).

      If not met, deny renewal.

Clinical guidelines do not recommend routine opioid use for painful diabetic neuropathy. Guideline guidance identifies four first-line drug classes — tricyclic antidepressants (TCAs), serotonin‑norepinephrine reuptake inhibitors (SNRIs), gabapentinoids, and sodium channel blockers — and advises using those treatments and other non‑opioid options before considering opioids for diabetic neuropathic pain.Opioids are not recommended for treating pain associated with painful diabetic neuropathy because of the high risks of addiction and other complications.

Although short‑term pain reduction with opioids has been demonstrated in patients with painful diabetic neuropathy, there are no randomized long‑duration trials showing clinically meaningful improvement in both pain and function needed to offset the long‑term risks. Therefore, long‑term opioid therapy for neuropathic pain is not supported by long‑duration randomized trials demonstrating meaningful improvement and should generally be avoided.

    Provider Requirements and Authorization Rules

    Prior Authorization

Prior authorization required for short-acting opioids

Prior authorization is required for short-acting opioids prescribed for neuropathic pain. Approvals are issued for up to 6 months and quantity is limited to a maximum of 50 MME per day, subject to meeting all coverage criteria.

    • Approval length: up to 6 months (initial and renewal).
    • Quantity limit: up to 50 MME per day.
    Step Therapy

Step therapy: opioids are third-line after required trials

Opioids are positioned as third-line therapy; the member must have adequate trials of first-line classes (TCAs, SNRIs, gabapentinoids, sodium channel blockers) before opioid approval. Documentation must show trials of two agents in each of these four classes or documented contraindications.

    • First-line classes: tricyclic antidepressants (TCAs); serotonin‑norepinephrine reuptake inhibitors (SNRIs); gabapentinoids; sodium channel blockers.
    • Required trials: two agents in each class (or documented contraindications).
    Documentation Required

Documentation must show funded diagnosis and required trials

Documentation submitted with the request must demonstrate a funded diagnosis of neuropathic pain and show adequate trials (or documented contraindications) to the required non‑opioid agents; for renewals include documentation of reduced pain and/or improved function and an opioid risk assessment (eg PDMP verification, ORT).

    • Funded diagnosis of neuropathic pain must be documented.
    • Adequate trials (or contraindications) to two agents in each of the four first‑line classes must be documented.
    • For renewals, document reduction in pain and/or improved function and opioid risk assessment (eg PDMP verification, ORT).
    Denial Risk

Non–short-acting opioid requests will be denied

Requests for non–short-acting opioids do not meet the criteria and will be denied.

    • Criterion #1 requires the request be for a short‑acting opioid; if not met, deny.
    Denial Risk

Denial risk if funded diagnosis not documented

Requests that lack documentation supporting a funded diagnosis of neuropathic pain will be denied.

    • Criterion #2 requires documentation of a funded diagnosis of neuropathic pain; if not met, deny.
    Denial Risk

Denial if required trials/contraindications are not documented

Requests will be denied if the member has not had adequate trials (or documented contraindications) to two agents in each of the four recommended non‑opioid drug classes.

    • Criterion #3 requires adequate trials of two agents per class (TCAs, SNRIs, gabapentinoids, sodium channel blockers); if not met, deny.
    Denial Risk

Renewal denied if initial criteria, benefit, or risk assessment not documented

Renewal requests will be denied if the initial criteria were not met, if there is no documented reduction in pain and/or improved function, or if there is no documented opioid risk assessment (eg PDMP verification, ORT).

    • Renewal requires that all initial criteria were met.
    • Renewal requires documented reduction in pain and/or improved function.
    • Renewal requires documented opioid risk assessment (eg PDMP verification, ORT).

    Required Trials of Non-Opioid Therapies

Trials of multiple agents and classes required before opioids (third-line).

Member must complete adequate trials of multiple agents and classes before opioids are approved (opioids are third-line):

    Conservative treatment requirements

    • Class requirement: Adequate trials (or documented contraindications) of two agents in each of the four recommended first-line drug classes (tricyclic antidepressants; SNRIs; gabapentinoids; sodium channel blockers).

If not met, opioid request is denied.

Placement in therapy: Opioids are reserved as third-line therapy and are not recommended for routine use in painful diabetic neuropathy.

Tramadol and tapentadol are limited in chronic use; tramadol is preferred over other opioids if an opioid is used.

    Class 1Tricyclic antidepressants (TCA)
    Class 2Serotonin–norepinephrine reuptake inhibitors (SNRIs)
    Class 3Gabapentinoids
    Class 4Sodium channel blockers
inv-15: Adequate trial duration — Guideline-consistent trial period of approximately 12 weeks at an effective dose before being considered a failure.
Recommended trial durationApproximately 12 weeks at an effective dose before being considered a failure
PurposeDetermine inadequate pain reduction or intolerable side effects before switching class
ApplicationUsed to define adequate trials of first-line agents prior to opioid consideration

    Clinical Background and Guideline Context

Clinical guidance recommends several non‑opioid pharmacologic classes as first‑line treatments for neuropathic pain: tricyclic antidepressants (TCAs), serotonin‑norepinephrine reuptake inhibitors (SNRIs), gabapentinoids, and sodium channel blockers. Trials of these agents are typically titrated to an effective dose and continued for about 12 weeks before being considered a treatment failure; if inadequate response or intolerable side effects occur, a trial of a different class is recommended.

    Not Covered / Denied Scenarios

Requests for routine, long‑term opioid therapy for neuropathic pain without documented benefit and an opioid risk assessment are not covered. Approvals for short‑acting opioids are limited, conditional, and require prior authorization; renewal requires continued documented reduction in pain and/or improved function and evidence of risk assessment (for example, PDMP verification or an opioid risk tool). In the absence of these demonstrations of benefit and risk mitigation, routine long‑term opioid therapy for neuropathic pain is not covered.
