OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria for Belimumab

    Initial and Renewal Criteria

Covered when ALL of the following are met (staged decision tree):

    Initial approval cluster

    • Diagnosis: Patient has active autoantibody-positive SLE with baseline assessment of SLE disease activity using a validated functional assessment tool OR has active lupus nephritis with renal biopsy within the previous 6 months to 2 years
    • Provider: Prescription written by (or in consultation with) a rheumatologist or nephrologist
    • Age: For SLE: patient age >5 years; for certain lupus nephritis pathways: patient age >18 years referenced
    • Concurrent therapy: Patient is currently receiving standard of care treatment for SLE or lupus nephritis and documentation member will continue receiving standard therapy
    • Concomitant biologics/CYC: Patient is not currently on other biologic therapy or intravenous cyclophosphamide
    • CNS lupus: No documentation of severe active central nervous system lupus
Renewal for SLE: For SLE renewal: member has been evaluated by a rheumatologist or nephrologist within the prior 6 months (or is scheduled for evaluation within 3 months) AND SLE disease activity has improved as assessed by a validated functional assessment tool
Renewal for LN: For lupus nephritis renewal: member age >18 years AND documentation or physician attestation of at least 50% reduction in proteinuria after 6 months of therapy AND continued receipt of standard therapy

Belimumab is not recommended and requests should be denied when there is documentation of severe active central nervous system (CNS) lupus. The clinical rationale in the policy specifies that belimumab has not been evaluated in this population and is contraindicated in combination with other biologic products; therefore, requests that would involve concomitant use of another biologic agent should be denied. Additionally, monitor and assess for depression and suicidality before and during treatment, as these events have been reported in trials of belimumab.

Operational denial criteria include the following scenarios: the prescription is not written by or in consultation with a rheumatologist or nephrologist (deny); the member does not meet the age thresholds (age <= 5 years for SLE or, where specified for LN renewal, age <= 18 years) (deny); baseline diagnostics or activity assessments are missing as required (for SLE, no baseline validated functional assessment tool; for LN, no renal biopsy within the prior 6 months to 2 years) (deny); the patient is currently on other biologic therapy or receiving intravenous cyclophosphamide (deny); or the patient is not currently receiving standard of care therapy for SLE or LN (deny).

    Provider Actions and Documentation Requirements

    Prior Authorization

Prior authorization required; contingent on staged criteria

Prior authorization is required for belimumab; approval is contingent on meeting the staged decision tree criteria including prescriber specialty, diagnosis (active autoantibody‑positive SLE with baseline assessment OR active lupus nephritis with recent renal biopsy), age thresholds, receipt of standard therapy, no concurrent biologic or IV cyclophosphamide, and absence of severe active CNS lupus.

    • Approval is granted only if all applicable criteria in the staged decision tree are met (see Criteria #2–#8 and #10).
    • Approval length: up to 6 months.
    Step Therapy

Must be receiving and continue standard-of-care therapy

The patient must be currently receiving standard-of-care treatment for SLE or lupus nephritis as a prerequisite for approval; lack of ongoing standard therapy leads to denial.

    • Criteria #6 requires the patient to be currently receiving standard of care treatment for SLE; Criteria #10 for renewal requires documentation the member will continue receiving standard therapy.

    Standard Therapy and Definitions

    Standard therapy (concomitant treatments)
Standard therapy expectationPatient is currently receiving and will continue to receive standard-of-care treatment for SLE or LN
Examples noted in policy contextConcomitant conventional treatments are referenced as the ongoing standard-of-care agents for SLE or LN (policy does not list specific agents in criteria text)
Prescriber roleApproval contingent on documentation that patient is receiving standard therapy as part of prior authorization requirements
Exclusion if absentRequests may be denied if standard therapy is not being provided or continued

    Background

Belimumab (Benlysta) is a B-lymphocyte stimulator (BLyS)–specific inhibitor approved for use in patients with active, autoantibody-positive systemic lupus erythematosus (SLE) aged 5 years and older receiving standard therapy, and for adults with active lupus nephritis receiving standard therapy. The policy notes reported risks of depression and suicidality, which should be assessed prior to and monitored during treatment.

  1. If documentation of current or continued standard therapy is not provided, the request is denied.
  2. Documentation Required

Required documentation: baseline disease activity and LN biopsy/response

Include baseline SLE disease activity assessment and other supporting documentation with the request: a validated functional assessment tool for SLE, or for lupus nephritis a renal biopsy within the prior 6 months to 2 years when baseline activity assessment is not present; for renewal of LN provide physician attestation of ≥50% reduction in proteinuria after 6 months.

    • Baseline assessment of SLE disease activity using a validated functional assessment tool is required for SLE (Criteria #3).
    • If SLE baseline assessment is not present, documentation of active lupus nephritis with a renal biopsy within the previous 6 months to 2 years is required (Criteria #4).
    • For LN renewal, documentation or physician attestation of at least a 50% reduction in proteinuria after 6 months of therapy is required (Renewal #4).
    Denial Risk

Denial risks: prescriber, age, missing documentation, excluded therapies

Requests are at risk of denial if prescriber specialty, age, or required documentation criteria are not met, or if excluded concurrent therapies or severe CNS lupus are present.

    • Deny if the prescription is not written by (or in consultation with) a rheumatologist or nephrologist (Criteria #2).
    • Deny if member age is ≤5 years for SLE (Criteria #5) or ≤18 years where age >18 is required for certain LN renewal criteria (Criteria #9/Renewal #4).
    • Deny if baseline SLE activity assessment or required renal biopsy documentation is missing (Criteria #3–#4).
    • Deny if the patient is currently on other biologic therapy or intravenous cyclophosphamide (Criteria #7), or if there is documentation of severe active CNS lupus (Criteria #8).
    • Deny renewal if documentation that the member will continue receiving standard therapy is not provided (Criteria #10).
