    Coverage Criteria

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Initial Review Criteria

Covered when ALL of the following initial review criteria are met:

    ALL of the following

    • Both therapies are prescribed by the same provider, OR, if there are multiple prescribers, both prescribers are aware of the concomitant therapy as evidenced by clinical notes.
    • ONE of the following

      • Concomitant therapy is deemed medically necessary as evidenced by the prescribers' clinical notes.
      • The patient has had a trial and failure on single therapy.
    • The necessity for continued concomitant therapy and safety is periodically assessed, as evidenced in clinical notes.
    • Therapy combination is applicable

      • Benzodiazepine and stimulant concomitant therapy.
      • Benzodiazepine and long-acting opioid concomitant therapy.
      • Non-benzodiazepine sedative and long-acting opioid concomitant therapy.

    Continuation of Therapy Criteria

Continuation of therapy requires ALL of the following:

    ALL of the following

    • Patient met the initial review requirements.
    • Clinical response to therapy is submitted.

      Supporting documentation is required.

    • Dosage and administration do not exceed the FDA-approved maximum for the patient's indication.
    • If the requested dose exceeds the FDA-approved maximum, supporting documentation is required.

    Provider Actions and Documentation

    Prior Authorization

    Initial Concomitant Therapy Authorization

Prior authorization is required for concomitant therapy, with authorization issued for six months. The applicable combinations are benzodiazepine with a stimulant, benzodiazepines with long-acting opioids, and non-benzodiazepine sedatives with long-acting opioids.

    • Benzodiazepine and stimulant concomitant therapy
    • Benzodiazepines and long-acting opioids concomitant therapy
    • Non-benzodiazepine sedative and long-acting opioids concomitant therapy
    • Authorization length: six months
    Step Therapy

    Single-Therapy Trial and Failure

Medical necessity for concomitant therapy may be established when the patient has undergone a trial of single therapy and failed to achieve adequate treatment, as documented in the clinical record.

    • Document the single-therapy trial and failure supporting the need for concomitant therapy.
    Documentation Required

Clinical Notes Required for Initial Review

Initial review documentation must show that both therapies are prescribed by the same provider. If multiple prescribers are involved, clinical notes must show that both prescribers are aware of the concomitant therapy; the notes must also establish medical necessity or document the single-therapy trial and failure.

    • Same provider prescribing both therapies, or clinical notes evidencing awareness by both prescribers.
    • Prescribers' clinical notes documenting medical necessity or trial and failure on single therapy.
    Documentation Required

    Continuation Supporting Documentation

For continuation authorization, submit documentation showing the patient's clinical response to therapy and periodic assessment of the continued necessity and safety of concomitant therapy. Dosage and administration must not exceed the FDA-approved maximum for the patient's indication unless supporting documentation is submitted for the requested higher dose.

    • Clinical response documentation is required.
    • Clinical notes must support periodic assessment of continued necessity and safety.
    • Supporting documentation is required when the requested dose exceeds the FDA-approved maximum for the indication.
    Denial Risk

Missing Awareness or Medical Necessity Documentation

Authorization may be denied when the required prescriber relationship or awareness is not documented, or when clinical notes do not establish medical necessity for concomitant therapy.

    • Missing documentation that the same provider prescribes both therapies or that multiple prescribers are aware of the concomitant therapy.
    • Missing medical-necessity documentation and no documented trial and failure on single therapy.
    Denial Risk

    Continuation Requirements Not Met

Continuation authorization may be denied when the patient did not meet the initial review requirements, required clinical-response documentation is missing, or the requested dosage exceeds the FDA-approved maximum without supporting documentation.

    • Patient did not meet the initial review requirements.
    • Clinical response to therapy was not submitted.
    • Dose exceeds the FDA-approved maximum without supporting documentation for the patient's indication.

    Policy Background

This policy addresses concurrent use of specified medication combinations that require coordinated prescribing and ongoing safety monitoring. The combinations include benzodiazepines with stimulants, benzodiazepines with long-acting opioids, and non-benzodiazepine sedatives with long-acting opioids. Coverage review considers documented medical necessity, prescriber awareness, periodic assessment of continued need and safety, and appropriate dosing.

    Initial Authorization

    Initial Authorization

Initial therapy is considered when all of the following requirements are met:

    Initial authorization criteria

    • Prescriber coordination

      • Both therapies are prescribed by the same provider.
      • If there are multiple prescribers, both prescribers are aware of the concomitant therapy, as evidenced by clinical notes.
    • Medical necessity

      • Concomitant therapy is medically necessary, as evidenced by prescribers' clinical notes.
      • Medical necessity is established by trial and failure on single therapy.
    • Ongoing need and safety assessment: The necessity for continued concomitant therapy and safety is periodically assessed, as evidenced in clinical notes.

    Continuation of Concomitant Therapy

    Continuation of Concomitant Therapy

Continuation requires all of the following:

    Continuation of concomitant therapy

    • The patient met initial review requirements.
    • Clinical response to therapy is submitted, with supporting documentation.
    • Dosage and administration do not exceed the FDA-approved maximum for the patient's indication.FDA-approved maximum
    • Supporting documentation is provided if the requested dose exceeds the FDA-approved maximum.

Documentation is required when the requested dose exceeds the FDA-approved maximum.
