OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Authorization and Documentation Requirements

    Prior Authorization

    Clinical Prior Authorization Required

Non-preferred colony-stimulating factor products require clinical prior authorization. The requested medication must have an FDA-approved indication, and the patient must be within the FDA-approved age limits.

    • Non-preferred products include Fulphila, Fylnetra, Granix, Neulasta, Nivestym, Releuko, Rolvedon, Ryzneuta, Stimufend, Udenyca, Zarxio, and Ziextenzo.
    • Preferred products are Leukine, Neupogen, and Nyvepria.
    Step Therapy

    Preferred-Product Step Therapy

Before approval of a non-preferred medication, the patient must have tried at least one preferred medication for the FDA-approved indication.

    • Preferred products: Leukine, Neupogen, and Nyvepria.

    Policy Background

Colony-stimulating factors support neutrophil production and are used to reduce or treat severe neutropenia. This policy establishes coverage and prior authorization requirements for preferred and non-preferred products when used for cancer-related indications, severe chronic neutropenia, AIDS-related neutropenia, or neutropenia associated with chemotherapy or acute exposure to myelosuppressive radiation. Coverage is subject to the applicable FDA-approved indication, age requirements, and clinical criteria.

    Initial Therapy

    Initial therapy

Initial approval requires all of the following:

    Initial approval requirements

    • The requested medication has the FDA-approved indication, and the patient is within the FDA-approved age limits.
