Submission and Processing Requirements
Submission requirements — include member ID and send originals
Write the Aetna member ID number on each itemized receipt and at the top of each page of the form; mail the completed form and the original itemized receipts to PO Box 981106 El Paso, TX 79998-1106 or fax them with originals to 1-866-474-4040. Make copies for your records; Aetna will not return submitted documents.

Device exclusions and limits — fitness tracker limit and exclusions
Fitness trackers that focus only on tracking health metrics (steps, heart rate, sleep) are covered but limited to one per member per plan year; golf trackers and smartwatches (for example, Apple® and Google watches) are excluded. Submit an itemized receipt and describe how you are using the tracker.
