Modifier 99 is a placeholder modifier used when more modifiers apply to a single procedure than the claim form or electronic field allows. The AMA classifies it under Provider Services and Ambulatory Service Center Modifiers and directs that it be used when two or more additional modifiers apply, typically placed first when multiple modifiers are required. CMS/MAC guidance (e.g., Noridian) expands on operational use: when more than four modifiers are needed on the same claim line, providers place modifier 99 in the modifier field (Item 24D on the CMS-1500) and list all actual modifiers as a narrative in Item 19 or the electronic equivalent. The CMS Claims Processing Manual and MAC instructions emphasize that the narrative must clearly show each modifier so adjudicators can determine which modifiers affect payment. Modifier 99 itself does not directly change reimbursement; payment is determined by the actual modifiers and supporting documentation. However, misuse—such as failing to include the narrative or not following payer/MAC sequencing rules—can lead to denials or incorrect adjudication. Payer and MAC practices vary (some require 99 to be first, others have specific positions), so always confirm local payer or MAC guidance before using modifier 99.
