Modifier -2P is a Category II performance‑measure exclusion appended to a CPT Category II (quality) code when the provider did not perform the measured action because of patient‑related reasons (for example, patient refusal, financial barriers, or religious objection). The AMA/AAPC/ACEP definitions state that -2P indicates patient reasons prevented the action and that using it excludes the patient from the measure denominator, not from payment processes. Category II codes are optional and used for quality reporting rather than reimbursement; they carry no RVUs and the modifier itself does not change fee schedule payment. By reporting the appropriate Category II code with -2P, providers document a valid patient‑driven exclusion so quality reporting systems and payers can remove that patient from the denominator and avoid penalizing clinicians for unmet measures due to patient choice or circumstances. Documentation of the patient reason (e.g., refusal, cost, religious objection) should be present in the medical record, and the modifier should only be used when the specific quality measure allows a patient‑reason exclusion for that Category II code.
