    Clinical & Coding Specifications

    Clinical Context

A 78-year-old patient with symptomatic bradycardia and prior transvenous pacemaker complications presents for removal and replacement of a complete dual‑chamber leadless pacemaker system. The patient has recurrent device‑related infection and lead malfunction with prolonged pauses and presyncope despite medical management. Preoperative evaluation includes device interrogation, chest imaging, echocardiography to assess cardiac anatomy, and review of anticoagulation. The procedure is performed in a cardiac catheterization lab or hybrid operating room under conscious sedation or general anesthesia. Vascular access is obtained percutaneously, imaging guidance (fluoroscopy and/or intracardiac echocardiography) is used to visualize device position and to safely snare and remove the existing leadless pacemaker components. Following explantation, a replacement dual‑chamber leadless pacemaker is positioned and deployed, device function and thresholds are verified, and hemostasis is achieved. The patient is monitored post‑procedure for bleeding, vascular complications, rhythm stability, and signs of infection before discharge to the inpatient ward or same‑day discharge per institutional protocol.
