This administrative rule amends Oregon rules governing designation of yellow fever vaccination centers and the provision/purchase and use of the uniform stamp used to validate International Certificates of Vaccination; it affects prescribers in Oregon who seek or hold designation to administer yellow fever vaccine.
The definition of 'uniform stamp' and related rule language were changed to remove language that the Oregon Health Authority provides the stamp and to require approved providers to purchase a stamp that meets Authority specifications.
Section (4) of OAR 333-045-0440 that previously stated the Authority orders and mails an official uniform stamp was deleted and replaced with direction that approved providers must purchase a stamp meeting Authority specifications.

Requirements and conditions for prescribers to be designated and to use the uniform stamp:
Application, Approval, and Stamp Procurement
Apply, receive approval, and procure uniform stamp (prescriber requirements)
Submit a completed Yellow Fever Uniform Stamp application (in the Authority’s form) signed and dated, together with a copy of your CDC Yellow Fever Vaccine Course certificate of completion, by mail or email to the Oregon Health Authority. The Authority may deny an application that does not satisfy the rule requirements. If approved, obtain/purchase a uniform stamp that meets the Authority’s specifications as identified in the approval notification; designation is valid only for the named prescriber and lasts three years. Maintain all control measures in the application agreement and reapply when designation expires or is revoked.
- Only a prescriber may apply (per 42 CFR Part 71.3).
- Applicant must submit CDC Yellow Fever Vaccine Course certificate of completion with the application (chunk 7).
- Submit the completed application and certificate by mail or electronic mail to the Authority (chunk 7).
- The Authority may deny an application if requirements are not satisfied (chunk 7).
- After approval, providers must obtain/purchase and use a stamp that meets Authority specifications as identified in the approval notification (chunks 8, 6).
- Designation is valid for three years and only for the prescriber named on the application; reapply if expired or revoked (chunk 7, 8).
- Maintain all control measures and requirements in the application agreement; failure to re-certify when requested will block ordering of yellow fever vaccine until re-certification is complete (chunk 8).
