Requests for Commercial and Qualified Health Plan members will be denied as not covered.

Medicare Advantage and related products: For Medicare Advantage, One Care, and Senior Care Options (SCO) members: Mass General Brigham Health Plan follows CMS guidance (NCDs, LCDs, LCAs, and Medicare manuals) for medical necessity determinations. When CMS guidance is not available, Mass General Brigham medical policies are used; if Medicare Advantage criteria are not met for One Care/SCO members, MassHealth criteria are applied.
