Monitor payer policy activity
Defines Mass General Brigham Health Plan medical necessity and coverage stance for DefenCath (taurolidine with heparin) catheter lock solutions for hemodialysis members across plan types.
Updated prior authorization requirements and added variation for One Care and SCO members; reformatted policy.
Coverage Criteria
Product-specific coverage
Coverage and prior authorization rules vary by product:
Requests for Commercial and Qualified Health Plan members will be denied as not covered.
Use CMS national and local coverage determinations and Medicare manuals as the primary source for medical necessity for these members.
Failure to obtain required prior authorization for ACO members may lead to claim denial.
Clinical evidence considerations
Evidence context informing medical necessity:
Consideration of trial limitations is required when applying evidence to clinical decisions.
Clinical use should consider existing institutional infection-prevention protocols; evidence does not support added benefit or cost-effectiveness of DefenCath over chlorhexidine-coated caps.
DefenCath (taurolidine with heparin) is not a covered service for Commercial and Qualified Health Plan members. Requests for coverage for these members will be denied as not covered.
Randomized trials and observational studies demonstrate that taurolidine/heparin catheter lock solutions reduce catheter-related bloodstream infections (CRBSIs) and may improve inflammatory markers and dialysis adequacy, but the evidence does not demonstrate an added clinical benefit or cost-effectiveness when DefenCath is used in addition to existing prevention protocols such as chlorhexidine-coated catheter caps. The available data therefore do not support routine use of DefenCath as a superior or cost-effective adjunct to those measures.

Provider Actions & Authorization
Prior authorization required for Mass General Brigham ACO; One Care/SCO follow CMS/MassHealth variations
Prior authorization is required for members of the Mass General Brigham ACO; requests for ACO members must follow the MassHealth Drug List criteria (Table 66: Antibiotics and AntiInfectives - Injectable). One Care and SCO members follow CMS guidance for medical necessity determinations, with MassHealth criteria applied when Medicare Advantage criteria are not met; the updated PA table notes variations for One Care and SCO members.
- Mass General Brigham ACO: PA required; use MassHealth Drug List Table 66 when submitting requests.
- One Care and SCO: follow CMS (NCDs/LCDs/LCAs); if Medicare criteria not met, MassHealth criteria apply.
- Refer to the updated PA table (policy effective May/Jan 2026) for specific submission details.
Consider alternative preventive strategies; DefenCath not shown superior to chlorhexidine-coated caps
Evidence recognizes multiple infection-prevention strategies and states that while taurolidine/heparin locks reduce CRBSI rates, there is no evidence demonstrating added benefit or superior cost-effectiveness of DefenCath compared with chlorhexidine‑coated catheter caps; consider existing institutional prevention protocols before using DefenCath.
- Randomized trials and observational studies support efficacy of taurolidine/heparin locks but do not show superiority over chlorhexidine-coated caps.
- Evidence does not support added benefit or cost-effectiveness of DefenCath when used in addition to chlorhexidine-coated catheter caps.
Documentation references required for prior authorization (CMS guidance and MassHealth Drug List)
For prior authorization submissions, use CMS guidance (NCDs, LCDs, LCAs, and Medicare manuals) for Medicare Advantage, and the MassHealth Drug List (Table 66: Antibiotics and AntiInfectives - Injectable) for Mass General Brigham ACO members as the basis for documentation of medical necessity.
- Medicare Advantage: base determinations on NCDs, LCDs, LCAs, and Medicare manual documentation.
- ACO (Mass General Brigham): apply MassHealth Drug List criteria (Table 66) when submitting PA documentation.
Commercial and Qualified Health Plan coverage exclusion — requests will be denied
DefenCath is not a covered service for Commercial and Qualified Health Plan members; any requests for these members must be denied as not covered.
- Do not submit PA requests for Commercial or Qualified Health Plan members expecting coverage—requests will be denied as not covered.
ACO members: prior authorization required — failure to obtain PA may lead to denial
For Mass General Brigham ACO members, prior authorization is required per the policy; failure to obtain the required prior authorization may result in denial of the service.
- Ensure PA is obtained using MassHealth Drug List criteria (Table 66) for ACO members prior to service.
- Ad hoc updates in Jan/May 2026 revised PA requirements—confirm current PA table when requesting authorization.
Background
DefenCath is a catheter lock solution composed of taurolidine combined with heparin, formulated to reduce catheter-related bloodstream infections (CRBSIs) in patients receiving chronic hemodialysis. Clinical trials (including the phase 3 LOCK IT-100 trial) and observational studies report reductions in CRBSI rates and improvements in inflammatory status and dialysis adequacy. Mechanistically, taurolidine provides antimicrobial activity when instilled as a lock solution and is typically used in combination with heparin to maintain catheter patency; however, questions remain about generalizability of trial results and how DefenCath compares with or adds to other infection-prevention strategies.
