Capital Bluecross considers thermal capsulorrhaphy investigational and therefore not covered for the management of joint instability. Providers should verify member benefit programs and eligibility, as identification of a code does not guarantee coverage.
Available evidence does not support the use of thermal capsulorrhaphy. Comparative studies are few and do not show efficacy for treating shoulder instability, and case series report high rates of unsatisfactory outcomes and complications. Reported adverse outcomes include capsular necrosis, loss of capsular and glenohumeral ligament integrity, chondrolysis, nerve damage, and failure resulting in recurrent instability; overall, the evidence does not demonstrate positive health outcomes.

Background
Thermal capsulorrhaphy uses applied thermal energy to induce collagen shrinkage in the joint capsule and ligaments with the intent of reducing capsular volume and restoring soft-tissue tension. Historically this has been performed with various energy sources, and contemporary procedures commonly use radiofrequency-based devices to deliver controlled thermal energy. The technique was developed primarily for glenohumeral (shoulder) instability but has also been explored for other joints.
