There is insufficient evidence to support a general conclusion concerning the health outcomes or benefits associated with this procedure; primary efficacy endpoints were not met in sham-controlled randomized trials (EMPOWER and ReCharge).

Intra-abdominal vagus nerve blocking therapy is considered investigational in all situations, including but not limited to the treatment of obesity. Procedure code 64999 is listed as investigational and, therefore, not covered under this policy. Identification of a code in this section does not denote coverage; coverage is determined by the member's benefit plan.

The evidence for vagus nerve blocking therapy includes two sham-controlled randomized trials. The predetermined primary efficacy outcome — a ≥10% difference between groups at 12 months — was not met in either trial. EMPOWER showed a 1% difference in excess weight loss at 12 months; ReCharge showed an 8.5% difference, which reached statistical significance only in a post hoc analysis and may not be clinically meaningful per the original trial design. Longer-term and post hoc analyses are limited by potential bias (missing data and unblinding). Overall, the evidence is insufficient to determine meaningful health outcome benefits for this technology.
