The evidence for vagus nerve blocking therapy includes two sham-controlled randomized trials. The predetermined primary efficacy outcome — a ≥10% difference between groups at 12 months — was not met in either trial. EMPOWER showed a 1% difference in excess weight loss at 12 months; ReCharge showed an 8.5% difference, which reached statistical significance only in a post hoc analysis and may not be clinically meaningful per the original trial design. Longer-term and post hoc analyses are limited by potential bias (missing data and unblinding). Overall, the evidence is insufficient to determine meaningful health outcome benefits for this technology.
