Monitor payer policy activity
Policy governing the medical coverage stance for actigraphy (wrist/ankle activity monitoring to infer sleep/wake) for BCBS Rhode Island members, including Medicare Advantage and commercial products.
No material clinical or coverage changes in this revision.
Coverage Determination
General coverage determination
Actigraphy coverage stance
This exclusion does not apply when actigraphy is used as a component of portable sleep monitoring; when used as a component it should not be separately reported.
This policy excludes the use of actigraphy when it is employed as the sole technique to record and analyze body movement to evaluate sleep disorders. The exclusion applies to actigraphy used alone to infer sleep/wake from movement. The exclusion does not apply when actigraphy is incorporated as a component of portable sleep monitoring; in that context actigraphy should not be separately reported.
For Commercial Products, actigraphy used as the sole technique to record and analyze body movement for evaluation of sleep disorders is considered not medically necessary. CPT code 95803 (actigraphy testing, recording, analysis, interpretation and report) is listed as not medically necessary for Commercial Products.
Billing and Codes
| 95803 | Actigraphy testing, recording, analysis, interpretation and report (minimum of 72 hours to 14 consecutive days of recording) |
Provider Requirements and Operational Notes
Prior Authorization
Prior authorization is not applicable for this policy.
Provider Action
No specific provider action placeholder was provided in the source. Providers should follow standard documentation and billing practices for actigraphy services and consult payer resources or provider relations for questions about claim submission.
Typical Device Use and Documentation
When actigraphy is used, patient bed/rise times (eg, lights out and lights on) should be entered into the device/software from daily patient sleep logs or by patient-activated event markers to support accurate interpretation of recorded movement.
- Document duration of recording (typically minimum 72 hours up to 14 consecutive days).
- Record source of bed/rise time data (sleep log or patient event marker).
Denial Condition / Not Medically Necessary
Actigraphy used as the sole technique to record and analyze body movement to evaluate sleep disorders is not covered for Medicare Advantage Plans and is not medically necessary for Commercial Products. When actigraphy is provided as a component of portable sleep monitoring, it should not be separately reported.
- Medicare Advantage Plans: Actigraphy (95803) not covered when used as the sole technique to evaluate sleep disorders.
- Commercial Products: Actigraphy (95803) not medically necessary when used as the sole technique to evaluate sleep disorders.
- When actigraphy is part of portable sleep monitoring, do not report actigraphy separately.
Background
Actigraphy refers to the assessment of body movement activity patterns using a device, typically worn on the wrist or ankle, with onboard sensors and algorithms that infer periods of sleep (absence of movement) and wake (movement). Devices are usually worn continuously and data are analyzed by software algorithms to estimate sleep onset, total sleep time, and other sleep metrics.
Actigraphy has been used for research purposes for more than two decades as an outcome measure in sleep disorders research and in clinical evaluation of conditions such as insomnia, circadian rhythm disorders, central hypersomnolence, and pediatric sleep disturbances. Comparisons with polysomnography and sleep diaries have shown variable agreement; actigraphy may reliably estimate some measures (for example sleep onset and total sleep time in certain populations) but has limitations in differentiating sleep from wake in patients with disturbed sleep.
For billing and coding context, CPT code 95803 describes actigraphy testing including recording, analysis, interpretation and report, with a recording duration of minimum of 72 hours to 14 consecutive days.
Definitions and Typical Parameters
OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.
