Use of actigraphy as the sole technique to record and analyze body movement for the evaluation of sleep disorders is excluded. For Medicare Advantage members, actigraphy used alone is considered not covered because the evidence is insufficient to demonstrate an improvement in net health outcome. This exclusion does not apply when actigraphy is used as a component of portable sleep monitoring; in that situation actigraphy should not be reported separately.
For Commercial Products, actigraphy when used as the sole technique to record and analyze body movement (including for evaluation of sleep disorders) is considered not medically necessary because evidence is insufficient to show improved net health outcome. CPT 95803 is listed as not medically necessary/not covered when billed for actigraphy as the sole technique.

Data capture and recording practices
When actigraphy is used, patient bedtimes (lights out) and rise times (lights on) are usually entered from daily sleep logs or by patient-activated event markers; proprietary device software then calculates sleep periods and reports sleep parameters.
- Devices are typically worn continuously (usually on nondominant wrist) for at least 24 hours, with activity usually recorded for 3 days to 2 weeks.
- Proprietary software analyzes activity data and reports parameters such as sleep onset/offset, sleep latency, total sleep duration, and wake after sleep onset.
Denial risk when actigraphy is sole technique
Actigraphy billed as the sole technique to record and analyze body movement for evaluation of sleep disorders is considered not covered for Medicare Advantage and not medically necessary for Commercial Products; CPT 95803 is listed as not covered/not medically necessary.

Background
Actigraphy is the assessment of body movement activity patterns using a device typically worn on the wrist or ankle; computer algorithms interpret the recorded activity to infer periods of sleep and wake. Devices are usually worn continuously and have been used in research and clinical settings for more than two decades to evaluate conditions such as insomnia, circadian rhythm disorders, hypersomnolence, and sleep disturbances in children. Accuracy compared with polysomnography varies by population and by the severity of sleep disturbance.
