Monitor payer policy activity
This policy governs the medical coverage stance for actigraphy (wrist/ankle activity monitoring to infer sleep/wake) for Blue Cross Blue Shield - Rhode Island members, including Medicare Advantage and Commercial plan distinctions.
Coverage Criteria
Actigraphy as sole technique
Coverage stance for actigraphy used as sole technique
Applies to Medicare Advantage (not covered) and Commercial Products (not medically necessary).
Actigraphy as component of portable sleep monitoring
Actigraphy when used as component of other testing
Separate reporting of actigraphy is not allowed in this context.
The MEDICAL CRITERIA section for this policy is Not applicable.
Actigraphy when used as the sole technique to record and analyze body movement (for example, to evaluate sleep disorders) is considered not covered for Medicare Advantage and not medically necessary for Commercial Products because evidence is insufficient to show improved net health outcomes. When actigraphy is used as a component of portable sleep monitoring, it should not be separately reported.
Coding
| 95803 | Actigraphy testing, recording, analysis, interpretation and report (minimum of 72 hours to 14 consecutive days of recording) |
Provider Actions
Prior authorization not applicable
Prior authorization is not required for actigraphy under this policy.
Document bedtimes, rise times, and use proprietary software for analysis
Enter patient bedtimes (lights out) and rise times (lights on) into the actigraphy software using daily patient sleep logs or patient-activated event markers; proprietary software is then used to calculate sleep parameters (sleep onset/offset, latency, total sleep duration, wake after sleep onset).
- Use daily patient sleep logs or event markers to record bed and rise times.
- Ensure downloaded actigraphy data are processed with the device-specific proprietary software to generate sleep parameter reports.
Denial risk: standalone actigraphy (CPT 95803) not covered
Claims for actigraphy when used as the sole technique (including CPT 95803) are considered not covered for Medicare Advantage and not medically necessary for Commercial Products; actigraphy used as a component of portable sleep monitoring should not be separately reported.
- Do not bill CPT 95803 as a covered standalone service for Medicare Advantage or Commercial Products under this policy.
- If actigraphy is part of portable sleep monitoring, do not report it separately.
Definitions
Background
Actigraphy assesses body movement using a wearable device (typically wrist or ankle) and applies proprietary algorithms to infer sleep and wake periods; when provided clinically, documentation commonly includes patient sleep logs or event markers and software-derived sleep parameters. For coding, CPT 95803 (Actigraphy testing: recording, analysis, interpretation and report) is listed as not covered/not medically necessary, with a required recording period of minimum of 72 hours to 14 consecutive days. When actigraphy is incorporated into portable sleep monitoring, it is not reportable as a separate service.
OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.
