Background and Rationale
Non-contact low-frequency ultrasound (NLFU) delivers mechanical energy in the kilohertz range without direct probe contact, typically transmitted through a saline mist or other noncontact medium. It is proposed to promote wound healing through mechanical effects including microbubble production and facilitation of debridement and cleansing, and devices such as the MIST Therapy System have received FDA 510(k) clearances for wound cleansing/debridement.
NLFU is intended to be used as an adjunct to standard wound care. Primary clinical trial endpoints in the evidence base include incidence and time to complete wound closure and pain control; however, BCBSNC has determined that available evidence is insufficient to conclude NLFU is medically necessary for wound treatment, which is reflected in the policy designation of the therapy as investigational.
