Coverage for voretigene neparvovec-rzyl requires absence of certain pre-existing ocular and systemic conditions that could affect treatment safety or effectiveness. Specifically, the patient must not have any pre-existing eye conditions or complicating systemic diseases that could alter ocular function or prohibit the planned procedure (examples include malignancies requiring orbital radiotherapy or leukemia with central nervous system/optic nerve involvement). Medical record documentation of the clinical assessment addressing these conditions is required for authorization.
Requests will be considered not medically necessary when required documentation is absent or when disqualifying conditions are present. Prior authorization must include medical record evidence of genetic confirmation of biallelic RPE65 mutations, assessment showing viable retinal cells (OCT and/or ophthalmoscopy criteria), and documentation of no intraocular surgery within the prior 6 months. Failure to supply these items or documentation of disqualifying conditions (for example, advanced retinopathy from diabetes or sickle cell disease, or acquired/congenital immunodeficiency) may result in denial.

Required Clinical Documentation — What to Submit
Ensure the following specific clinical items are included in the prior authorization submission to support medical necessity: age, genetic testing results confirming biallelic RPE65 mutations, assessment of viable retinal cells (OCT thickness >100 µm in posterior pole OR ≥3 disc areas without atrophy/pigmentary degeneration in posterior pole OR remaining visual field within 30° of fixation by III4e isopter or equivalent), history of intraocular surgery, presence/absence of contraindicating ocular or systemic disease, and proposed dosing consistent with FDA label.
