Coverage Criteria
Medical necessity documentation
Covered when ALL of the following are met
If criteria not met, submit a Letter of Medical Necessity (LOMN) requesting a Clinical Exception (Individual Consideration).
LOMN must contain the items listed. Incomplete documentation may result in prior authorization denial or incomplete claims processing.
Failure to include the consult note and physician signature may result in prior authorization denial or incomplete claims processing.
If the patient does not meet all of the listed coverage criteria, submit a Letter of Medical Necessity (LOMN) requesting a Clinical Exception (Individual Consideration).
