Conclusions from the section and summary of evidence
No studies identified for cervical/thoracic indications
The IG-MLD (mild®) devices and procedure are intended for central lumbar spinal stenosis and are not intended for use near the lateral neural elements. The device instructions and policy state the procedure is contraindicated for disc procedures and cannot address nerve root compression or disc herniation; it is performed dorsal to the dura under fluoroscopic guidance and targets resection of hypertrophied ligamentum flavum and small laminar bone only.
Because the technique relies on percutaneous fluoroscopic guidance and specialized single-use tools, it is limited in scope and should not be used when lateral element decompression or discectomy is required.
There is no evidence assessing use of image-guided minimally invasive decompression for cervical or thoracic spinal stenosis. The policy review found no clinical studies to inform conclusions about these anatomic regions.
As a result, IG-MLD for cervical or thoracic indications lacks clinical data to support safety or effectiveness and cannot be recommended based on the current evidence base.
Image-guided minimally invasive spinal decompression is considered experimental / investigational and is not covered under this policy.
The available evidence is insufficient to determine the effects of IG-MLD (mild®) on health outcomes. Although a large ongoing randomized trial and smaller studies report early improvements in pain and function compared with epidural steroid injections, concerns about lack of blinding, potential bias, and absence of placebo or open-decompression comparators limit confidence in benefit.
Because trials with appropriate control groups and longer, blinded follow-up are lacking, the evidence does not establish efficacy or comparative effectiveness; therefore IG-MLD remains experimental/investigational.

Actions for providers and billing guidance
Archive claim volume / contact BCBSKS
There were few claims being received for this service at the time of archiving. If there are questions about coverage of this service, contact Blue Cross and Blue Shield of Kansas Customer Service, your professional or institutional relations representative, or submit a predetermination request.
Coverage depends on member contract
Coverage for procedures referenced in this policy depends on the member's contract and any applicable state or federal mandates. Inclusion of codes in the policy is informational only and does not guarantee coverage or reimbursement. Verify member benefits with BCBSKS prior to scheduling or billing.
Diagnostic imaging documentation required
Verify radiologic documentation demonstrating ligamentum flavum hypertrophy > 2.5 mm on preoperative MRI or CT when applicable. Include imaging reports in the medical record to support medical necessity when submitting predetermination or claims.
Predetermination / benefits verification recommended
Submit a predetermination (prior authorization / benefits verification) request when coverage is uncertain. Predetermination is recommended to confirm member benefits and any applicable medical necessity requirements before providing the service.
Prior conservative therapy should be documented
Document trial of prior conservative therapy (e.g., physical therapy, home exercise programs, and oral analgesics) in the medical record, as many studies and selection criteria required failure of standard therapies before intervention.
Other provider actions
Provider action not otherwise specified in this document. For additional guidance on billing, coding, or clinical questions not addressed here, contact BCBSKS Customer Service or submit a predetermination request.
Clinical background
Spinal stenosis is narrowing of the spinal canal or neural foramina that can compress the spinal cord or nerve roots and commonly causes neurogenic claudication—leg pain or weakness worsened by standing or walking and relieved by sitting. Lumbar stenosis is a frequent cause of disability in older adults and may be managed conservatively (physical therapy, exercise, analgesics) or with surgical decompression.
Conventional open decompression (eg, laminectomy, laminotomy) is established but can be associated with postoperative morbidity and potential spinal instability; IG-MLD (mild®) is a percutaneous, image-guided technique developed to debulk hypertrophied ligamentum flavum and achieve central canal decompression with less tissue disruption.
