Coverage criteria for manual wheelchair
Initial medical necessity criteria
Covered when ALL of the following are met and documented on the CMN:
Refer to Section 1B fields on the CMN (supplier, patient, diagnosis, estimated length of need, percent of day in wheelchair) for required entries.
Responses must be recorded using the Section 2 checkboxes on the CMN.
See Section 3 - handwritten physician signature and date required on the CMN.
The CMN form does not explicitly state exclusion language on its face; however, coverage decisions rely on completed CMN fields. In practice, if the CMN fails to document key items—specifically the Section 2 medical necessity responses demonstrating that, without a wheelchair the patient would be bed- or chair-confined—or omits the required physician attestation and signature, the case may be excluded from coverage consideration. Providers should ensure all required CMN items are completed to avoid denial for insufficient documentation.
Section 2 asks whether the patient is able to ambulate with crutches or a walker. If the CMN documents that the patient is able to ambulate with crutches or a walker and no other Section 2 criteria (such as confinement or other mobility-limiting conditions) are checked, the manual wheelchair may be considered not medically necessary. Providers must answer this question and document any additional qualifying limitations to support coverage.

Provider responsibilities and form completion
Required form completion
Required form completion — Supplier and physician sections must be fully completed. The form must include supplier information, patient demographics, diagnosis (ICD-10) codes, estimated length of need (months or 99 = lifetime), initial/revised certification dates, and percent of day spent in wheelchair. If physician section is blank, supplier must ensure physician information is completed.
- Supplier name, address, phone, NPI
- Patient name, DOB, ID number, height, weight, county
- Diagnosis codes (ICD-10), comma separated
- Estimated length of need (1-99 months; 99 = lifetime)
- Initial and Revised Certification Dates
- Percent of day patient spends in wheelchair
- Itemized charges/attachments if applicable
Clinical status questions
Clinical status questions must be answered in Section 2. The form asks whether the patient is bed- or chair-confined without a wheelchair, ambulatory with crutches or walker, and about specific clinical conditions that support wheelchair need (quadriplegia, fixed hip angle, trunk cast/brace, excessive extensor tone, need to rest recumbent ≥2 times/day, knee flexion limitation, significant lower-extremity edema requiring elevating leg-rest, reclining back ordered, need for non-standard arm height). Mark Yes or No for each item and provide any required details.
- Is the patient bed- or chair-confined without a wheelchair? (Yes/No)
- Is the patient able to ambulate with crutches or walker? (Yes/No)
- Quadriplegia / fixed hip angle / trunk cast or brace / excessive extensor tone / need to rest recumbent ≥2x/day (Yes/No)
- Cast/brace or musculoskeletal condition preventing 90° knee flexion or significant lower-extremity edema requiring elevating leg-rest or reclining back (Yes/No)
- Need for arm height different than non-adjustable arms (Yes/No)
DME-specific medical necessity details
DME-specific necessity nodes
Medical necessity is determined by documented clinical responses on the CMN (Section 2):
Documented via Section 2 checkbox ('Is the patient's condition such that without the use of a wheelchair, he/she would otherwise be bed or chair confined?').
Select applicable checkbox(es) from Section 2.
Select applicable checkbox(es) from Section 2.
Required documentation and attachments
Provide complete, signed CMN with supporting attachments
Ensure the CMN is complete and signed by the physician and include supporting attachments: list diagnosis codes (ICD-10), estimated length of need in months, percent of day spent in the wheelchair, and the physician attestation with a handwritten signature and date. Attach itemization of items/charges if applicable.
- Record ICD-10 diagnosis codes and Estimated length of need (1–99 months) on the CMN.
- Document percent of day spent in the wheelchair on Section 1B.
- Physician attestation in Section 3 requires a handwritten signature and date; signature stamps are not acceptable.
Confirm CMN fields and physician attestation are completed
Duplicate emphasis: confirm the CMN includes ICD-10 diagnosis, the estimated length of need (months), and that Section 2 clinical responses are completed; obtain the physician's handwritten attestation and signature in Section 3.
- Verify ICD-10 codes and estimated length of need are entered in Section 1B.
- Confirm all Section 2 medical necessity checkboxes are answered to support confinement/mobility limitations.
- Obtain physician handwritten signature and date on Section 3 (no signature stamps).
Key definitions used in this policy
Rental, purchase and replacement rules
| Equipment | Rental vs. Purchase |
|---|---|
| Manual wheelchair | |
| Determined by payer rules (the CMN requests estimated length of need in months which may inform rental vs. purchase decisions); the form does not specify the payer's rental vs. purchase policy |
Background
Manual wheelchairs are prescribed to provide mobility for patients who cannot safely ambulate and who would otherwise be confined to a bed or chair without the device. The CMN collects specific clinical details (Section 2) — confinement status, ambulation ability, neurologic or musculoskeletal limitations, need to recline or elevate legs, and arm height requirements — that determine whether a manual wheelchair is clinically indicated and which seating/positioning features may be required.
Items not covered
This CMN addresses manual wheelchairs only. Power wheelchairs are not covered by the criteria captured on this form and are not addressed by the questions in Section 2; providers seeking coverage for power mobility should use the appropriate documentation pathway for power wheelchairs.
