Policy/Criteria II
The policy states that safety and efficacy have not been established for several device- or procedure-based interventions for repair of nasal vestibular stenosis. Specifically, radiofrequency ablation (VivAer®), absorbable implants (Latera®), and nasal valve suspension surgery are considered unestablished and are not supported by this policy for repair of nasal valve compromise. Providers should not rely on these techniques to meet the covered indications described in the medically necessary criteria.
Procedures billed with CPT 30468 or CPT 30469 do not support the coverage criteria in this policy and are therefore considered not medically necessary for repair of nasal valve compromise. Claims for these codes are at risk for denial when submitted for nasal valve repair because the policy explicitly lists them as not supporting coverage.

Prior Authorization for Covered Repair (CPT 30465)
Prior authorization is required for repair of nasal vestibular stenosis when using CPT 30465. Prior authorization requests must demonstrate that all medical necessity criteria in this policy are met before the procedure is performed.

Required Clinical Documentation
Providers must submit focused clinical documentation to support medical necessity. Documentation should include history, focused physical exam findings, results of indicated diagnostic tests, and evidence of prior conservative therapy and response.
- Focused history and physical exam of the nose, nasal cavity, and nasopharynx (including laterality, time course, triggers, trauma, prior surgery, systemic conditions)
- Objective and/or documented test results as applicable: NOSE score, Cottle's maneuver, Bachman's maneuver, nasal decongestant testing, rhinomanometry, acoustic rhinometry, anterior rhinoscopy, nasal endoscopy, CT of paranasal sinuses, peak nasal airflow
- Documentation that other causes of obstruction were evaluated and ruled out (e.g., sinusitis, allergic/nonallergic rhinitis, nasal polyps, adenoid hypertrophy, nasopharyngeal masses, septal deviation, turbinate hypertrophy)
- Documentation of conservative therapy trials and response (e.g., nasal cones, external nasal dilator adhesive strips, six-week trial of nasal steroids, outpatient nocturnal trial of external stents or nasal dilators such as Breathe Right® strips, Max-Air Nose Cones® or Sinus Cones®)
- Clinical note showing that symptoms improve with Cottle maneuver or lateralization of the upper lateral cartilage
Background and Rationale
Nasal valve compromise (NVC), also called nasal valve collapse, is a narrowing of the nasal passage that produces obstructed breathing. It can result from structural weakness of the internal or external nasal valves due to prior surgery, trauma, chronic inflammation, congenital defects, or aging, and may be unilateral. Typical symptoms include difficulty inhaling, a sensation of congestion, and symptoms that worsen with certain positions or stimuli. The clinical evaluation for NVC focuses on identifying internal valve collapse (upper lateral cartilage) or external valve compromise (lower lateral cartilage) and assessing whether symptoms improve with diagnostic maneuvers such as the Cottle maneuver or with lateralization of the upper lateral cartilage; these findings, together with exclusion of other causes and failure of conservative therapy, form the basis for the policy’s medical necessity criteria for repair.
