Coverage Criteria for linezolid (Zyvox)
Initial Therapy — Covered when ALL of the following are met

Criteria #1 from policy: Non-tablet requests are referred to Medical Director.
See policy requirement for clinically documented infection and supporting lab culture.
If documented failure or contraindication is not provided, the request is denied (Criteria #5).
Initial approvals are for a course of therapy per indication; parenteral therapy should be reserved for inability to take oral meds or absorption issues.
Renewal Therapy
Renewal criteria and allowable extension rules:
If initial criteria not met, deny renewal.
If standard course completed, follow guidance below; for certain indications longer standard durations (e.g., 28 days) may apply.
If documentation supports continued treatment and for VRE or other documented needs, approve up to an additional 14 days. Continued treatment beyond standard course may require infectious disease specialist recommendation; absent adequate documentation, do not approve and forward to Medical Director if specialist recommendation is present.

Consider infectious disease consultation for therapy beyond standard durations; IV reserved for inability to take oral therapy or absorption issues.
Requests will be denied when documentation does not confirm a clinically documented infection or when the request lacks required evidence of failure or contraindication to preferred formulary agents. Specifically, if the request does not meet Criteria #4 for a confirmed diagnosis or strong suspicion of MRSA when applicable, or if Criteria #5 documentation (failure/contraindication to other preferred agents) is not provided, the request must be denied.
Linezolid is not supported as a preferred first-line agent absent documented bacterial susceptibility or evidence the member has failed or cannot take preferred formulary alternatives. The policy requires documentation that the member has failed or has a contraindication to other preferred agents before approval (Criteria #5); use of linezolid as routine first-line therapy for infections without confirmed susceptibility or without documented failure/contraindication is not supported and should be denied.

Prior authorization is required for linezolid (Zyvox). Approval is granted when the member has a clinically documented infection (bacteria confirmed) or an appropriate infection with strong suspicion of MRSA, and there is documentation the member has failed or has a contraindication to preferred formulary agent(s); initial approval authorizes one fill for a course of therapy.
Show failure or contraindication to preferred formulary agents
Document that the member has failed, or has a contraindication to, other preferred formulary agents for the type of MRSA infection being treated; approval requires this evidence of prior failure or contraindication.
- Criteria #5 requires documentation the member has failed or has contraindication to other preferred formulary agent(s) to treat this type of MRSA infection; without this documentation the request is denied.
Provide lab culture and clinical documentation of confirmed infection
Include supporting clinical documentation of a clinically documented infection and a laboratory culture showing an organism sensitive to linezolid; the policy specifies culture results demonstrating susceptibility are required for approval.

Denial risk: lack of confirmed infection or failure documentation

Background
Linezolid (Zyvox) is an oxazolidinone antibacterial with activity against Gram-positive organisms including MRSA and vancomycin-resistant Enterococcus faecium. It has 100% oral bioavailability, so parenteral therapy should be reserved for patients with impaired gastrointestinal absorption or inability to take oral medications. Risks of serious hematologic toxicity increase after >2 weeks of therapy and neurologic toxicity increases after >4 weeks, so prolonged use is discouraged and may warrant infectious disease consultation.
