Outpatient laboratory billing criteria

Billing and coverage rules for outpatient laboratory services:

Claims must include ordering and rendering provider information, appropriate and accurate ICD diagnosis codes (sufficient specificity), date of service, and place of service.

Codes must be selected based on testing attributes (method/technology), not clinical indication; if an approved PLA or MAAA code exists for the test, that code must be used.

If a test qualifies for an AMA-defined panel code, the panel code(s) must be used; if no appropriate panel code exists, either a single unit of 81479 or individual component codes may be used; only one unit of 81479 may be billed per test.

Proprietary/PLA codes may be used only for the specific test to which the code is assigned.

Code selection and unit limits

When available, use approved Proprietary Laboratory Analyses (PLA) or MAAA codes; if a test qualifies for an AMA-defined panel code the panel code(s) must be used; if no appropriate panel code exists, either a single unit of 81479 or individual component codes may be used — but only one unit of 81479 may be billed per test.

Panel codes required when the test meets AMA panel definitions.
If no panel code exists, bill either one unit of 81479 or the individual components.

NCCI/PTP and MUE compliance

Bill in alignment with NCCI procedure-to-procedure (PTP) edits and Medically Unlikely Edits (MUEs); do not exceed MUE maximum units and use modifiers only when appropriate (Modifier 59 only if no other appropriate modifier applies).

If codes are subject to NCCI PTP edits, follow the edit (mutually exclusive procedures cannot be billed together).
If codes have an MUE, units billed must not exceed the MUE maximum.
Modifiers should be used only when appropriate; Modifier 59 is limited to situations where no other appropriate modifier exists.
