- Age: Patient is at least 18 years of age
- Vitamin A supplementation: Patient is receiving supplementation with vitamin A at the recommended daily allowance
- Concomitant TTR agents: Must not be used in combination with other transthyretin (TTR) reducing or stabilizing agents (e.g., inotersen, tafamidis, vutrisiran, eplontersen, etc.)
hATTR polyneuropathy-specific criteria
- Definitive diagnosis: Patient has a definitive diagnosis of hATTR amyloidosis as documented in a proband with suggestive findings (including imaging or histopathology findings of amyloidosis) and a heterozygous pathogenic (or likely pathogenic) variant in TTR identified by molecular genetic testing
- Polyneuropathy evidence (at least 2)
  - Subjective symptoms: Subjective patient symptoms are suggestive of neuropathy
  - Nerve conduction studies: Abnormal nerve conduction studies are consistent with polyneuropathy
  - Neurological exam: Abnormal neurological examination is suggestive of neuropathy

Concomitant Therapy & Exclusions

Exclusions: Onpattro must not be used in combination with other transthyretin (TTR) reducing or stabilizing agents (examples listed include inotersen, tafamidis, vutrisiran, eplontersen). The policy also requires that other causes of neuropathy be excluded before attributing peripheral neuropathy to hATTR, and treatment is not covered for patients who have received an orthotopic liver transplant (OLT).
