Coverage Criteria by Indication

Initial Approval - Universal

Covered when ALL of the following are met

Universal criteria: Patient must have a contraindication, intolerance, or failure to Alymsys AND Mvasi prior to consideration of another bevacizumab product; patient is at least 18 years of age unless otherwise specified; no recent history of hemoptysis (≥2.5 mL); no surgical procedure within the preceding 28 days and no surgical wound that has not fully healed.

Ampullary Adenocarcinoma

Covered when ALL of the following are met

Ampullary Adenocarcinoma: Bevacizumab used in combination with a fluoropyrimidine-based regimen (e.g., 5-fluorouracil or capecitabine) for intestinal-type ampullary adenocarcinoma; AND used as first-line therapy for metastatic disease OR used for disease progression.

Adult Central Nervous System (CNS) Cancers

Covered when specific diagnosis and use-case criteria are met

CNS symptomatic management: Used as a single agent for symptomatic mass effect, radiation necrosis, or brain edema in listed CNS tumor types (e.g., circumscribed glioma; primary CNS lymphoma; meningiomas; brain or spine metastases; primary spinal cord tumors; medulloblastoma; glioblastoma/gliosarcoma; H3-mutated high-grade glioma/HGAP/PXA WHO grade 3; IDH-mutant astrocytoma; IDH-mutant, 1p19q codeleted oligodendroglioma; intracranial or spinal ependymoma).

Initial prior authorization duration differs (12 weeks) and renewal is not permitted for symptom management per policy.

CNS recurrent/progressive disease: Used for recurrent or progressive disease in specified CNS tumor subtypes (e.g., IDH-mutant, 1p19q codeleted oligodendroglioma WHO Grade 3; glioblastoma/gliosarcoma; H3-mutated high-grade glioma/HGAP/PXA WHO grade 3; IDH-mutant astrocytoma WHO Grade 3 or 4). Permitted as single agent OR in combination with carmustine, lomustine, or temozolomide after failure of bevacizumab monotherapy or per listed scenarios; single-agent use allowed for intracranial or spinal ependymoma after prior radiation.

Other CNS uses: Additional covered uses include combination temozolomide + irinotecan for recurrent medulloblastoma; single-agent use for surgically inaccessible meningiomas when radiation is not possible; and single-agent use for NF2 vestibular schwannomas with hearing loss.

Cervical Cancer

Covered when ALL of the following are met

Cervical cancer: Patient has persistent, recurrent, or metastatic cervical cancer (adenocarcinoma, adenosquamous, or squamous cell) AND bevacizumab is used in combination with paclitaxel plus either cisplatin, carboplatin, or topotecan; OR used in combination with pembrolizumab, paclitaxel, and cisplatin or carboplatin when tumor PD-L1 CPS ≥1 by an FDA-approved or CLIA-compliant test; OR used with atezolizumab plus paclitaxel and cisplatin or carboplatin. Bevacizumab may be continued as maintenance when specified. Single-agent subsequent therapy or use in NECC with specified combinations is allowed.PD-L1 CPS >=1 for pembrolizumab-containing combinations

Colorectal Cancer (CRC)

Covered when ALL of the following are met

Colorectal cancer: Will not be used as adjuvant treatment. Bevacizumab used in combination with a fluoropyrimidine-based regimen as first-line or subsequent therapy for metastatic, unresectable, or advanced CRC; OR used with irinotecan as initial treatment for unresectable metastatic disease (with prior FOLFOX/CapeOX within past 12 months as specified); OR used with irinotecan-based therapy or with trifluridine/tipiracil as subsequent therapy after progression through available regimens. Also permitted as primary treatment for specified rectal staging scenarios when resection is contraindicated after total neoadjuvant therapy or neoadjuvant/definitive immunotherapy.

Appendiceal Neoplasms and Cancers

Covered when ALL of the following are met

Appendiceal neoplasms: Used as neoadjuvant therapy or for metastatic/peritoneal-only disease in combination with a fluoropyrimidine-based regimen; OR used as initial therapy for recurrent or metastatic peritoneal-only or extraperitoneal disease in combination with a fluoropyrimidine; OR used as subsequent therapy in combination with a fluoropyrimidine or irinotecan-based regimen or with trifluridine/tipiracil after progression through available regimens.

Endometrial Carcinoma (Uterine Neoplasms)

Covered when ALL of the following are met

Endometrial carcinoma recurrent or advanced: Used for recurrent disease (not for certain locoregional recurrences without prior radiation) as single-agent subsequent therapy after progression on cytotoxic chemotherapy; OR used in combination with carboplatin and paclitaxel and continued as single-agent maintenance; OR used as primary/adjuvant therapy for stage III-IV disease in combination with carboplatin and paclitaxel and continued as maintenance.

Hepatocellular Carcinoma (HCC)

Covered when ALL of the following are met

Hepatocellular carcinoma: Used in combination with atezolizumab as first-line therapy for unresectable or metastatic HCC; OR used as subsequent therapy after progression on systemic therapy provided the patient has not previously received bevacizumab or a checkpoint inhibitor.

Peritoneal Mesothelioma (PeM)

Covered when ALL of the following are met

Peritoneal mesothelioma: Used as adjuvant therapy following cytoreductive surgery (CRS) + HIPEC in combination with pemetrexed and cisplatin or carboplatin when high-risk features are present; OR used as first-line, subsequent, or in combination with atezolizumab per specified conditions. High-risk features include Ki-67 >9%, nodal metastasis, thrombocytosis, PS=2, PCI >17, cc score >1, biphasic/sarcomatoid histology, or bicavitary disease.

May also be used for pericardial mesothelioma and tunica vaginalis testis mesothelioma.

Pleural Mesothelioma (PM)

Covered when ALL of the following are met

Pleural mesothelioma: Used in combination with pemetrexed and either cisplatin or carboplatin as induction prior to surgery for stage I epithelioid disease, as first-line therapy, or as subsequent therapy (including rechallenge) depending on prior immunotherapy or pemetrexed response.

May also be used for pericardial mesothelioma and tunica vaginalis testis mesothelioma.

Non-Squamous Non-Small Cell Lung Cancer (NSCLC)

Covered when specified biomarker, line-of-therapy, and combination regimen conditions are met

NSCLC first-line: Used for recurrent, advanced, or metastatic non-squamous NSCLC as first-line therapy in combination with specified partners (e.g., erlotinib for EGFR exon 19/L858R, carboplatin + paclitaxel, or pemetrexed-based regimens depending on biomarker status). Actionable biomarker testing (EGFR, KRAS, ALK, ROS1, BRAF, NTRK, MET, RET, NRG1, ERBB2/HER2) is recommended to guide therapy.PD-L1 >=1% in some contexts

NSCLC subsequent/maintenance: Used as subsequent therapy in combination with atezolizumab/carboplatin/paclitaxel (excluding prior PD-1/PD-L1 recipients) or as continuation maintenance therapy (single agent or combined with pemetrexed/atezolizumab) following response/stable disease after first-line systemic therapy; continuation after progression on erlotinib+bevacizumab allowed under specific conditions with T790M negative disease.

NSCLC biomarker testing:

Ovarian, Fallopian Tube, and Primary Peritoneal Cancer

Covered when tumor type, histology, and line-of-therapy criteria are met

Ovarian and related cancers: Covered uses include malignant stage II-IV sex cord-stromal tumors and epithelial ovarian, fallopian tube, or primary peritoneal cancers. Bevacizumab may be used in platinum-sensitive or -resistant disease with specified combinations (e.g., carboplatin + paclitaxel), as maintenance, neoadjuvant/adjuvant therapy, or combined with PARP inhibitors depending on BRCA/HR status and prior therapies.

Ovarian / Fallopian Tube / Primary Peritoneal cancer-related epithelial subtypes

Covered when specific histologies and combination/sequence conditions are met

Epithelial subtypes and sequences: Use in specified epithelial histologies (mucinous, clear cell, carcinosarcoma, endometrioid, high-grade serous, low-grade serous) for combinations with paclitaxel + carboplatin for rising CA125 or recurrence when no prior chemotherapy; maintenance therapy after primary therapy including bevacizumab; single agent or combination with PARP inhibitors depending on BRCA and HR status and histology-specific criteria.

Neoadjuvant use does not apply to low malignant potential or other non-invasive cancers such as ovarian borderline epithelial tumors; see policy for exclusions and histology-specific constraints.

Pediatric CNS cancers

Covered when indication- and age-specific conditions are met

Pediatric CNS indications: Recurrent or progressive disease in pediatric CNS: diffuse high-grade glioma (≤21 yrs) as single-agent palliation; medulloblastoma (3–21 yrs) as part of TEMR or MEMMAT regimens; optic pathway glioma (<18 yrs) as subsequent therapy to preserve vision; NF2 vestibular schwannomas (≥6 yrs) to preserve hearing; continuation after progression on bevacizumab also addressed.

Age and regimen-specific constraints apply.

Renal Cell Carcinoma (RCC)

Covered when ALL of the following are met

RCC coverage: Used in combination with interferon alfa for metastatic RCC; OR for relapsed or stage IV non-clear cell histology in combination with everolimus OR in combination with erlotinib for advanced papillary disease including HLRCC-associated RCC. When used as first-line for stage IV disease, disease must be M1 or unresectable T4 M0.

Respiratory papillomatosis

Covered when ALL of the following are met

Respiratory papillomatosis: Used as an adjunct to endobronchial therapy (e.g., surgical excision, debridement) for recurrent respiratory papillomatosis.

Small bowel adenocarcinoma

Covered when ALL of the following are met

Small bowel adenocarcinoma: Patient has advanced or metastatic small bowel adenocarcinoma AND bevacizumab is used in combination with a fluoropyrimidine-based regimen; used as initial therapy if pMMR/MSS OR as subsequent therapy if not previously given.

Soft tissue sarcoma (STS)

Covered when ALL of the following are met

STS: Covered as single agent for angiosarcoma OR in combination with temozolomide for solitary fibrous tumor.

Vaginal cancer

Covered when ALL of the following are met

Vaginal cancer regimens: Patient has recurrent or metastatic vaginal cancer AND bevacizumab used in combination with paclitaxel plus cisplatin, carboplatin, or topotecan as first-line or subsequent therapy; OR used with pembrolizumab plus paclitaxel and cisplatin or carboplatin when tumor PD-L1 CPS ≥1. Single-agent subsequent therapy also allowed.PD-L1 CPS >=1 for pembrolizumab-containing regimens

PD-L1 must be determined by an FDA-approved or CLIA-compliant test.

Vulvar cancer

Covered when ALL of the following are met

Vulvar cancer: Patient has advanced, recurrent, or metastatic vulvar cancer AND bevacizumab is used as first-line or subsequent therapy in combination with paclitaxel plus cisplatin or carboplatin OR with pembrolizumab plus paclitaxel and cisplatin or carboplatin. Bevacizumab (and pembrolizumab when used) may be continued as maintenance per policy.

Bevacizumab will not be used as part of adjuvant treatment for colorectal cancer. This policy permits bevacizumab for colorectal indications only in specified settings such as first-line or subsequent therapy for metastatic/unresectable/advanced disease in combination with fluoropyrimidine-based regimens, irinotecan-based regimens, or trifluridine/tipiracil as described elsewhere in the policy, but explicitly excludes adjuvant use.

Neoadjuvant use is allowed only in the specific disease and histology scenarios listed in the policy and does not apply to low malignant potential (borderline) or other non‑invasive tumors (for example, ovarian borderline epithelial tumors). Where neoadjuvant regimens are permitted, the policy specifies allowed combination partners and histology exclusions (see disease‑specific sections for details).

Medicare beneficiaries may be subject to separate Medicare coverage rules. National Coverage Determinations (NCDs), Local Coverage Determinations (LCDs), and Local Coverage Articles (LCAs) may apply and supersede this non‑Medicare guidance where applicable; providers should consult the Medicare Coverage Database and relevant MAC/LCD documents for Part B drug coverage requirements.

Requests that do not meet the policy’s universal criteria are considered not medically necessary. Examples include lack of documented prior intolerance, contraindication, or failure to Alymsys® and Mvasi® before considering another bevacizumab product; patients younger than 18 years when adult age is required; recent hemoptysis (defined as ≥2.5 mL of blood in sputum); or a surgical procedure within the preceding 28 days or an unhealed surgical wound. Exceptions or approvals outside these criteria require clear documentation meeting the plan’s prior authorization and renewal documentation standards.
