Initial Approval

Covered when ALL of the following are met:

Initial Eligibility: Member is at least 1 month of age.

Baseline Documentation: Documented baseline values for one or more of the following: weight-for-age z-scores (for members with growth failure), LDL-c, HDL-c, non-HDL-c, triglycerides, or AST and/or ALT.

Diagnostic Confirmation: Diagnosis of lysosomal acid lipase (LAL) deficiency confirmed by either biallelic pathogenic or likely pathogenic variants in the LIPA gene OR deficient LAL enzyme activity measured in peripheral blood leukocytes, fibroblasts, or dried blood spots.

Confirmation of LAL deficiency to establish eligibility for sebelipase alfa therapy

Diagnosis confirmed by genetic testing or enzymatic assay:

Genetic or Enzymatic Confirmation: Genetic testing demonstrating biallelic pathogenic or likely pathogenic variants in the LIPA gene.

Genetic or Enzymatic Confirmation: Deficient LAL enzyme activity measured in peripheral blood leukocytes, fibroblasts, or dried blood spots.

Renewal and Dose Escalation

Renewal allowed when ANY of the following are met (with additional requirements):

General Renewal Preconditions: Member continues to meet the indication-specific relevant criteria identified in the Initial Approval section; AND absence of unacceptable toxicity from the drug (examples include hypersensitivity reactions such as anaphylaxis).

Evidence of Clinical Benefit: Treatment has resulted in clinical benefit demonstrated by one or more of: improvement in weight-for-age z-scores, improvement in LDL-c, improvement in HDL-c, improvement in non-HDL-c, improvement in triglycerides, or improvement in AST and/or ALT.

Dose Escalation Criteria (pediatric & adult): Dose escalation to 3 mg/kg every other week is allowed for pediatric and adult members with suboptimal clinical response to the 1 mg/kg dose, defined by at least one of: poor growth, deteriorating biochemical markers (e.g., ALT, AST), and/or worsening lipid parameters (e.g., LDL-c, triglycerides).

Unacceptable toxicity to sebelipase alfa, including hypersensitivity reactions such as anaphylaxis, is a contraindication to continued therapy and may be a basis for denial of renewal requests. Renewal requires absence of such unacceptable toxicity as described in the policy.

Covered Indications

Confirmation of LAL deficiency to establish eligibility for sebelipase alfa therapy

Diagnosis may be confirmed by either of the following:

Genetic Confirmation: Detection of biallelic pathogenic or likely pathogenic variants in the LIPA gene.

Germline genetic testing of LIPA is acceptable.

Enzymatic Confirmation: Demonstration of deficient LAL enzyme activity in peripheral blood leukocytes, fibroblasts, or dried blood spots.

Enzymatic assay methods such as dried blood spot, leukocyte, or fibroblast testing are acceptable.

Member Eligibility Requirements

Prior authorization is required for Kanuma (sebelipase alfa). Initial authorization is valid for 6 months (180 days); subsequent renewals may be approved for 12 months when renewal criteria are met. Members must be at least 1 month of age at treatment initiation, and eligibility requires documented baseline clinical or laboratory values (for example, weight-for-age z-scores if growth failure, lipid panel components, or liver enzymes) and confirmation of diagnosis by either biallelic pathogenic or likely pathogenic LIPA variants or deficient LAL enzyme activity measured in peripheral blood leukocytes, fibroblasts, or dried blood spots.

Renewal Denial Triggers

Renewal may be denied for unacceptable toxicity (e.g., hypersensitivity reactions including anaphylaxis) or for lack of clinical benefit. Clinical benefit should be demonstrated by improvement in one or more of: weight-for-age z-scores (if applicable), LDL-c, HDL-c, non-HDL-c, triglycerides, or AST/ALT. Renewal may still be approved if dose escalation is used appropriately for suboptimal responders per the policy criteria.
