Reimbursement Criteria

Reimbursement criteria for pancreatic enzyme testing

Covered or not covered when ALL of the following criteria are met:

Policy applies to services billed on UB04 (CMS 1450) and CMS 1500 forms or electronic equivalents for UnitedHealthcare Medicare Advantage products.

ALL of the following

ONE of the following clinical presentations must be documented:
- Mild to severe epigastric pain (may radiate to the back)
- Nausea
- Vomiting
- Tenderness to palpation of the epigastrium
- Abdominal distention
- Hypoactive bowel sounds
- Fever
- Rapid pulse (tachycardia)
- Tachypnea
- Hypoxemia
- Hypotension
- Anorexia
- Diarrhea
- Cullen sign
- Grey Turner sign

Frequency limitation for serum lipase: no more than once per week for the initial determination of acute pancreatitis; the once-per-week limitation is defined as seven consecutive calendar days from the initial date of service, excluding the same date of service, and applies across all billing/rendering providers for the member.

Serum amylase (CPT 82150) — UnitedHealthcare will not consider separate reimbursement when submitted in conjunction with lipase for the same date of service.

Urine amylase (CPT 82150) — not reimbursed for the initial determination of acute pancreatitis.

Serum or urine trypsin/trypsinogen/TAP (e.g., CPT 83519) — not reimbursed for assessment, prognosis, or determination of acute pancreatitis.

Listed biomarker procedure codes (e.g., CPTs 83520, 83529, 84145, 86140) — not reimbursed for assessment, prognosis, or determination of acute pancreatitis.

This policy applies to UnitedHealthcare Medicare Advantage Plans and to services billed on UB04 (CMS 1450) and CMS 1500 forms (or their electronic equivalents). Providers are responsible for submitting accurate claims using the code(s) that correctly describe the services provided. UnitedHealthcare Medicare Advantage reimbursement policies use CPT, CMS, and other coding guidelines for definitional purposes only and do not guarantee reimbursement. Providers should follow applicable coding methodology, industry-standard reimbursement logic, regulatory requirements and their contracts or a member’s benefit coverage documents when billing.

Providers must submit accurate claims using codes that correctly describe services provided.
