Diagnosis of onychomycosis
Antifungal therapy has failed to resolve infection
NAAT (CPT 87480, 87481) may be reimbursed when the above criteria are met
ATR-FTIR spectroscopy (CPT 88749) is not reimbursed for screening, determination, or confirmation of onychomycosis
