Onychomycosis diagnostic testing coverage criteria
Criteria for consideration of reimbursement:
ALL of the following
- Diagnosis of onychomycosis
- Antifungal therapy has failed to resolve infection
- NAAT (CPT 87480, 87481) may be reimbursed when the above criteria are met
- ATR-FTIR spectroscopy (CPT 88749) is not reimbursed for screening, determination, or confirmation of onychomycosis
