Coverage criteria for Once in a Lifetime Procedures

UnitedHealthcare Community Plan will reimburse only one procedure per Code Family in a patient's lifetime, with specific exceptions when allowed modifiers are appended or when state-specific lists apply.

Reimburse one procedure per Code Family once in a patient’s lifetime; for example, one appendectomy code from the Appendectomy Code Family (44950, 44955, 44960, 44970) will be reimbursed only once.

Modifier exceptions: Procedures reported with modifier 53, 55, 56, or 58 may be considered separately for reimbursement when appropriate; refer to Discontinued Procedure, Split Surgical Package and/or Global Days policies for payment percentage rules.

Duplicate-claim handling: When duplicate codes from the Once in a Lifetime list are reported on multiple dates of service (excluding the same date of service), UnitedHealthcare Community Plan reimburses only one claim using a 'first in, first out' methodology; same-date duplicates are subject to duplicate-billing logic.

Same-date duplicates are subject to duplicate-billing logic. Modifiers such as 53, 55, 56, and 58 may allow separate consideration when appropriately appended (for example, modifier 53 for a discontinued procedure may be reported first and the full procedure reported later).
