Covered when ALL of the following rules are met:

- UnitedHealthcare Community Plan will follow CMS MUE values before applying any other Maximum Frequency Per (MFD) criteria; if no CMS MUE exists or it is not exceeded, the plan's Maximum Frequency Per policy applies.
- For MUE purposes, the same provider is the same individual rendering services reporting the same Federal Tax Identification number.
- If units of service reported on a claim line exceed the established MUE for the HCPCS/CPT code, all units on that claim line will be denied for professional and facility (CMS-1500 and UB-04) claims.
