Reimbursement Rules for Laboratory Services

Reimbursement for duplicate laboratory services

Rules determining when duplicate or repeat laboratory services are reimbursable.

ALL of the following

Only one laboratory service is reimbursable when identical or equivalent component codes are submitted for the same patient on the same date by the same individual provider (Same Individual Physician or Other Qualified Health Care Professional).
Separate consideration may be given for repeat procedures on the same day when reported with modifier 91.
Modifiers 59, XE, XP, XS, XU may be used to indicate distinct services (different species/strains or specimens from separate anatomic sites) and may allow reimbursement when properly appended.
Modifiers 76 and 77 are inappropriate for repeat laboratory services and will not result in separate reimbursement.

Multiple providers and duplicate services

Determines which provider is reimbursable when multiple parties submit the same laboratory service.

ANY of the following

Only one laboratory provider will be reimbursed when multiple individuals report duplicate services (includes independent labs, reference labs, referring labs, pathologists, physicians).
If Reference Laboratory and Non-Reference Laboratory submit duplicate services, only the Reference Laboratory is reimbursable.
If both purchaser (facility) and supplier bill duplicate services, only one is reimbursable unless modifier 59, XE, XP, XS, XU or 91 is appended.
If a pathologist and a treating physician report identical codes for the same patient/date, only the pathologist will be reimbursed unless an appropriate distinguishing modifier is appended.

Facility setting laboratory services

Rules for lab services performed in facility place of service codes.

ALL of the following

Manual and automated laboratory services submitted with facility POS (19,21,22,23,26,34,51,52,55,56,57,61) will not be reimbursable to independent/reference/non-reference providers — these are reimbursable to the facility.
Exception: molecular pathology, genomic sequencing, other molecular multianalyte assays, and proprietary laboratory analysis procedures may be reimbursable to non-facility labs when facility lab is not equipped and refers testing out.
UnitedHealthcare uses CMS NPFS PC/TC indicators 3 and 9 to identify services not reimbursable to independent/reference/non-reference providers in a facility setting.

Date of service rules

Defines the date of service used on claims for laboratory tests.

ALL of the following

Date of service is the specimen collection date; if collection spans 2 calendar days, the date collection ended is the DOS.
Exception for ADLTs and certain molecular pathology tests excluded from OPPS packaging: DOS may be the date the test was performed if five specified conditions are met (performed after hospital outpatient discharge; specimen collected during encounter; medically appropriate to collect at encounter; results do not guide treatment during encounter; test reasonable and medically necessary).

This policy defines UnitedHealthcare Medicare Advantage reimbursement methodology and billing rules for laboratory panels and related services billed on CMS 1500 forms. It covers when panel CPT/HCPCS codes must be billed instead of unbundled component codes, rules for venipuncture and specimen collection, handling and conveyance charges, place and date of service determination, duplicate or repeat laboratory services, and which provider types are eligible for reimbursement.

The policy applies to all Medicare Advantage products and services reported using the CMS 1500 (or electronic equivalent) and to all network physicians and other qualified health care professionals. It is intended to ensure claims are submitted accurately and that reimbursement is based on the code(s) that correctly describe the services provided.

Key operational points include: billing the HCPCS panel code when all components of a panel are performed (effective for dates of service on or after April 1, 2020); only one collection fee allowed per specimen type per encounter; HCPCS venipuncture code S9529 is non‑reimbursable (use CPT 36415 with the appropriate place of service); and a valid federal CLIA certificate identification number is required for reimbursement of clinical laboratory services on CMS 1500.

Do not bill S9529; use 36415 with correct POS

Do not bill HCPCS S9529 for venipuncture; report venipuncture using CPT 36415 and the appropriate place of service. Specimen collection code G0471 is reimbursable only when the specimen is collected from an individual in a skilled nursing facility or collected by a laboratory on behalf of a home health agency.

S9529 — non-reimbursable venipuncture code (do not bill).

G0471 — reimbursable only for specimen collection in SNF or when collected by lab for home health agency.

Billing Rule

Panel billing and unbundling

For dates of service on or after April 1, 2020, when all components of an HCPCS organ/disease-oriented panel are performed, providers must bill the HCPCS panel test code rather than unbundling into individual component codes; claims will be denied if the panel code is not billed.

Panel codes include 80047, 80048, 80051, 80053, 80061, 80069, 80076.
If all components of a panel are performed on the same date of service, bill the panel HCPCS code; do not submit individual component codes.

Duplicate services across providers

When duplicate laboratory services are submitted by multiple providers, only one provider will be reimbursed; exceptions require appropriate distinguishing modifiers such as 91, 59, XE, XP, XS, or XU. Reference laboratories appending modifier 90 are eligible for reimbursement; non-reference providers appending 90 are not.

Billing Rule

Facility vs non-facility POS reimbursement

Laboratory services performed in facility places of service (CMS POS 19, 21, 22, 23, 26, 34, 51, 52, 55, 56, 57, 61) are reimbursable to the facility and not to external independent, reference, or non-reference providers, except for certain molecular/genomic or proprietary assays when the facility refers testing out.

Facility POS are considered POS 19, 21, 22, 23, 24, 26, 31, 34, 51, 52, 55, 56, 57 and 61 for policy purposes.
Manual and automated tests billed with facility POS will not be reimbursable to independent/reference/non-reference providers — reimbursable to the facility.
