Reimbursement for discarded drugs and biologicals (modifier JW)

This policy governs how UnitedHealthcare reimburses providers for discarded amounts of drugs and biologicals from single-use vials or packages when reported with modifier JW; it applies to UnitedHealthcare Commercial and Individual Exchange benefit plans and affects physicians, hospitals, and other providers/suppliers submitting CMS 1500 or UB04 claims.

Reimbursement Criteria

Reimbursement criteria for discarded drugs/biologicals

Reimbursement is allowable when the discarded drug or biological originates from a single-use vial or single-use package, is reported with modifier JW on a separate claim line, and both administered and discarded amounts are documented in the medical record.

Discarded amount must originate from a single-use/dose vial (SDV) or other single-use package. The JW modifier is only permitted to identify discarded amounts from a single vial or single package; JW is inappropriate for multi-dose vials (MDVs).

Report the administered amount on one claim line and report the discarded amount on a separate claim line with modifier JW appended to the associated CPT/HCPCS code.

When more than one vial with different NDCs is used, report each NDC on a separate claim line with the appropriate units administered, and add an additional separate line indicating the discarded units with modifier JW.

Units must be billed in multiples of the dosage specified in the full CPT/HCPCS descriptor (bill in billing-unit multiples). The JW modifier applies only to discarded amounts and not to administered amounts.

When a single-use vial or single-use package is partially used and the remainder discarded, providers may report reimbursement for both the amount administered and the discarded amount (up to the vial/package amount shown on the manufacturer label). Append modifier JW to the CPT/HCPCS code on a separate line to report the discarded amount from a single-use vial or single-use package. Do not append JW for multi-dose vials or other disallowed scenarios. Follow CMS guidance to report the administered amount on one line and the discarded amount on a separate line with JW appended to the associated CPT/HCPCS code.

Billing Rule

JW modifier and billing unit rules

Do not append modifier JW when the administered dose is less than the billing unit. Instead, bill the administered amount by rolling the administered quantity UP to the next billing unit, and report the discarded quantity by rolling DOWN to the previous billing unit on a separate line with JW appended. Units must be billed in multiples that correspond to the CPT/HCPCS descriptor dosage (billing unit). Billing additional JW units that would duplicate or exceed the vial amount is not permitted and may result in overpayment.

If billing unit = 10 mg and 7 mg administered with 3 mg discarded: bill 1 unit (10 mg) for the administered dose; do NOT add a JW line for the 3 mg discarded.
If billing unit = 10 mg and 77 mg administered from a 100 mg SDV: report 8 units (80 mg) as administered on one line and on a separate line report 2 units (20 mg) with modifier JW to indicate the discarded amount.
Units billed must align with the CPT/HCPCS descriptor dosage; choose vial size consistent with minimizing wastage when possible.

Documentation Required

Units billed must be in multiples of the dosage specified in the CPT/HCPCS descriptor. The JW modifier applies only to discarded amounts and must not be used to represent administered amounts. Providers must document in the medical record both the quantity administered and the quantity discarded (when applicable), including vial sizes/NDCs used and calculations showing how billed units were derived. When multiple NDCs are used, document each NDC and the units administered from each vial and show the separate JW line(s) for discarded units.
