Primary Diagnosis Coding Requirements

Providers must code and submit diagnoses in accordance with the ICD-10-CM Official Guidelines for Coding and Reporting for the claim to be considered for reimbursement.

For UB-04 claims the principal diagnosis must be reported in Box 67 (or its electronic equivalent); for CMS-1500 claims the primary diagnosis is indicated via Box 24E diagnosis pointers (or electronic equivalent).

If an inappropriate diagnosis code is pointed to or linked as primary in Box 24E on a CMS-1500, only the claim line(s) associated with that diagnosis will be denied.

If an inappropriate principal diagnosis code is submitted as the principal diagnosis in Box 67 of the UB-04, the entire claim will be denied under this policy.

State Medicaid programs may have exceptions or customized 'Inappropriate Primary ICD-10 Diagnosis Codes' lists (for example Kansas and Rhode Island); providers must follow any state-specific guidance included in the policy attachments.
