Coverage Criteria and Reimbursement Rules

inv-01: Duplicate services and purchased diagnostics — reimbursement hierarchy and duplicate service rules

Duplicate laboratory services and reimbursement hierarchy

Only one laboratory service is reimbursable when Duplicate Laboratory Services are submitted from the Same Group Physician or Other QHP.

Separate consideration will be given to repeat procedures by the Same Group Physician or Other QHP when reported with modifier 91; modifier 91 is appropriate when repeat testing for the same patient on the same day is necessary to obtain subsequent results.

Only one laboratory provider will be reimbursed when multiple individuals report Duplicate Laboratory Services (this may include physicians, Other QHPs, Independent Laboratories, Reference Laboratories, Referring Laboratories or pathologists).

If a Reference Laboratory and a Non-Reference Laboratory Provider submit Duplicate Laboratory Services, only the Reference Laboratory service is reimbursable.

Laboratory services billed with modifier 90 by a Referring Laboratory are reimbursable only if a duplicate claim has not been received from an Independent Laboratory or Reference Laboratory; duplicate services are not reimbursable unless one laboratory appends modifier 91.

If a pathologist and a Physician Office Laboratory provider submit Duplicate Laboratory Services, only the pathologist's service is reimbursable unless the Physician Office Laboratory provider appends modifier 91.

If both purchaser and supplier who performed the service bill Duplicate Laboratory Services, only one service is reimbursable unless modifier 59, XE, XP, XS, XU or 91 is appended. UnitedHealthcare uses CMS NPFS PC/TC indicators 1, 6, and 8 to identify services eligible as Purchased Diagnostic Tests.

inv-02: Order and documentation requirements — required orders, signatures, and documentation for lab reimbursement

Documentation and ordering criteria

Diagnostic laboratory tests must be ordered by the treating physician or Other QHP and used in patient management; tests not ordered by the physician or Other QHP are not reasonable and necessary.

The physician's or Other QHP's documentation must clearly indicate the specific test(s) to be performed; vague directives such as 'run labs' or 'check blood' alone do not support intent to order.

Acceptable documentation of an order or intent includes: a signed order/requisition listing specific tests; an unsigned order/requisition plus an authenticated medical record supporting intent; an authenticated medical record supporting intent; or an electronic requisition when the laboratory can demonstrate receipt through a standardized process.

The medical record should include the documentation described above and a copy of the test results.

inv-03: Coverage criteria for laboratory services — UnitedHealthcare coverage and bundling rules for laboratory and pathology services

UnitedHealthcare coverage and bundling rules for laboratory and pathology services include the following criteria.

Tests must be ordered by the treating physician or Other QHP and documented with specific test(s) indicated (signed requisition, authenticated record, or standardized electronic order).

Facility-performed laboratory services (CMS POS 19,21,22,23,26,34,51,52,55,56,57,61) are reimbursable only to the facility; Independent, Reference or Referring laboratories will not be reimbursed for duplicate facility technical services except specified exceptions (e.g., reproductive lab procedures, molecular/genomic assays, proprietary PLAs).

When all component codes comprising an organ or disease-oriented panel (CPT 80047, 80048, 80050, 80051, 80053, 80055, 80061, 80069, 80074, 80076, 80081) are submitted by the Same Individual Physician or Other QHP on the same date, UnitedHealthcare will bundle them to the appropriate panel code; fewer than all components are considered individually. Panel configurations and precedence rules are defined in policy.

Only one collection fee for each type of specimen per patient encounter is allowed. Venipuncture codes 36415 and 36416 are reimbursed once per patient per date of service with 36415 bundling over 36416 when both submitted. Collection from implantable venous access devices and established catheters (36591/36592) follow bundling rules; when both reported, only 36592 is eligible. Code S9529 is non-reimbursable; G0471 is reimbursable only for skilled nursing facility collections or on behalf of a home health agency.

Place of service billed should reflect the location where the specimen was obtained (examples: POS 21/22 for hospital inpatient/outpatient, POS 11 for physician office, POS 81 if the laboratory drew the specimen on-site).

Reference Laboratories should append modifier 90 when billing laboratory services performed as outside/reference labs.

Facility-performed laboratory reimbursement limitation

Manual and automated laboratory services performed in a facility setting reported with facility Place of Service (POS) codes 19, 21, 22, 23, 26, 34, 51, 52, 55, 56, 57, or 61 are reimbursable to the facility only. When a facility obtains laboratory tests under arrangements with an Independent Laboratory, Reference Laboratory, or pathology group, only the facility may be reimbursed for those services. UnitedHealthcare uses CMS NPFS PC/TC indicators 3 and 9 to identify services not reimbursable to an Independent, Reference, or Non‑Reference Laboratory provider in a facility setting. Exceptions are made when the facility is not equipped and refers specialized services (e.g., reproductive laboratory medicine procedures, molecular pathology, Genomic Sequencing Procedures, other molecular multianalyte assays, and proprietary laboratory analyses) to an appropriate laboratory; in such cases the referring facility may not be expected to perform the service and the outside laboratory may be reimbursed. If both a facility and an Independent or Reference Laboratory report the same service on the same day for the same member, only the facility laboratory services may be reimbursed.

Appropriate repeat/distinct service modifiers (59, XE, XP, XS, XU, 91) are required to identify separate or repeat laboratory services and avoid denials for duplicate or serial tests when applicable.

Surgical pathology specimens for prostate needle biopsy are identified by submission of CPT codes 55700 or 55706 and should be coded with HCPCS G0416 rather than CPT 88305 when so identified (G0416 represents one unit regardless of number of specimens).
