Drug testing reimbursement criteria

UnitedHealthcare reimbursement rules for drug testing services billed on CMS-1500:

Presumptive drug testing codes represent all drug/Drug Class tests performed by that methodology and are reimbursable once per patient per date of service.

Definitive drug testing CPT codes 80320-80377 are considered non-reimbursable; report HCPCS G0480-G0483 or G0659 for definitive testing, which represent a per-day service covering the total number of different Drug Classes.

UnitedHealthcare will allow only one presumptive test and one definitive test per date of service per patient, regardless of the number of Drug Classes, specimen validations, or billing providers.

Specimen Validity Testing is included in the presumptive and definitive code descriptions and is not separately reimbursable when performed on the same date of service; it may be reimbursed separately only if performed distinctly separate from the drug test, appended with an appropriate modifier, and supported by documentation.

Do not bill more than one presumptive drug test and one definitive drug test per patient per date of service. Specimen validity testing performed on the same date of service is considered integral to the presumptive/definitive code descriptions and will be denied unless it was performed distinctly separate with an appropriate modifier and supporting documentation.

Court‑ordered or government‑funded testing denied

Drug testing services that are court-ordered or funded by a county, state or federal agency will be denied to healthcare professionals.
