NPWT in the outpatient setting or upon discharge is proven and medically necessary when a complete wound therapy program has been tried or considered and the indication-specific criteria below are met.

Complete wound therapy program: Documentation of evaluation, care, and wound measurements; application of dressings to maintain a moist wound environment; debridement of necrotic tissue if present; evaluation of and provision for adequate nutritional status; documentation by provider of indication for NPWT; documentation that open wound has not responded to conventional treatment after 30 days30 days

Venous leg ulcers: Limited evidence (single study cited in Hayes) suggests NPWT may increase likelihood of healing when compression has failed; guidelines recommend considering NPWT to prepare the wound for surgical closure if first-line compression therapy is ineffective.

Open surgical wounds / temporary abdominal closure: RCTs show mixed results: some trials (e.g., Moreno et al.) demonstrate reduced surgical site infections and other surgical site occurrences after laparotomy with NPWT, while Cochrane reviews report uncertainty for open abdomen outcomes; consider NPWT in patients with risk factors acknowledging heterogeneity of evidence.

Coverage considerations and criteria

Evidence- and guideline-derived considerations for the use of NPWTi-d and NPWT to help inform coverage decisions:

General evidence-based considerations: NPWTi-d has been associated with higher primary wound closure rates and lower complication rates versus NPWT or standard dressings in several meta-analyses, but the overall evidence is low-quality, heterogeneous, and largely retrospective; large randomized trials are needed to confirm benefits.

When NPWT/NPWTi-d is suggested by guidelines: Consider NPWT for stage III/IV pressure injuries that fail conventional therapy; consider NPWT to reduce wound size in postoperative diabetic foot wounds; IWGDF suggests not using NPWT in preference to best standard care for non-surgical diabetic foot ulcers.

Adjunctive/stepwise use before NPWT/NPWTi-d: Adjunctive modalities including NPWT should be used only after optimization of standard care (re-evaluate vascular status, infection control, offloading) and lack of expected improvement (for example, failure to achieve >50% wound area reduction after a minimum of 4 weeks).>50% area reduction after >=4 weeks

Contraindications to negative pressure wound therapy (NPWT) include presence of active bleeding or exposed vasculature within the wound, eschar or necrotic tissue, exposed bone, nerves, or organs, and malignancy present in the wound. The policy also lists presence of an open fistula to body organs or cavities in the wound vicinity as a contraindication. These conditions should prompt avoidance of NPWT and discontinuation of therapy if they occur.

The policy states there is insufficient clinical evidence to support routine use of NPWT on closed surgical incisions, including for disposable/single‑use NPWT systems. Existing studies are limited in size or at risk of bias, and further high‑quality prospective trials are needed to determine which surgical populations, if any, benefit from these devices.

For pilonidal disease, available randomized evidence does not show benefit of NPWT over conventional passive dressings for preventing surgical wound dehiscence or improving recovery. The policy therefore considers NPWT for pilonidal disease to have insufficient evidence to support routine use.

The National Institute for Health and Care Excellence (NICE) concluded that the VAC Veraflo (instillation) system shows promise for treating some acute infected or chronic wounds but that there is not enough good‑quality evidence to support routine adoption. NICE recommends further research comparing instillation therapy with NPWT alone to demonstrate clinically meaningful benefits.

The policy update revised the contraindication list to add anticoagulation therapy and to clarify infection‑related exclusions as uncontrolled soft tissue infection or untreated osteomyelitis within the vicinity of the wound. Large randomized trials and systematic reviews provide mixed results for incisional NPWT. Notably, the WHIST multicenter RCT found no significant reduction in deep surgical site infection or improvements in patient‑reported outcomes for lower limb fractures, and other trials (e.g., cesarean delivery RCTs) have not consistently shown benefit. These randomized data do not support routine use of incisional NPWT across all surgical settings.

Evidence for NPWT with instillation and dwell time (NPWTi‑d) is limited by heterogeneous, mostly retrospective studies and small case series. Systematic reviews report promising signals (higher primary closure rates, high pooled healing proportions), but overall study quality is low and results are heterogeneous; therefore NPWTi‑d is not supported as routine first‑line therapy without further high‑quality randomized trials.
