Coverage Criteria for NPWT

Chronic and neuropathic ulcers (pressure ulcers, diabetic foot ulcers, neuropathic ulcers)

Evidence indicates NPWT may be effective in selected chronic wounds and neuropathic/diabetic foot ulcers when used as adjunct therapy to improve wound bed and promote healing.

General evidence summary: Randomized trials and meta-analyses report improved granulation tissue formation, reduced wound area, higher healing rates, and lower risk of amputation for NPWT compared with moist dressings or standard care in diabetic foot ulcers and pressure injuries

Caveats include small trials, heterogeneous quality, and limited long-term follow-up

Pressure injuries (Stage III/IV): RCTs and guideline statements report NPWT increases granulation tissue formation and reduces wound dimensions for Stage III and IV pressure ulcers

Consider as early adjunct to promote wound bed preparation or until surgical closure is possible; evidence limited by small RCTs

Diabetic foot ulcers (DFUs): Systematic reviews and meta-analyses of RCTs demonstrate higher rates of complete healing, shorter healing times, greater reduction in ulcer area, and lower risk of major amputation with NPWT compared with moist wound care in selected DFU populations4-8 weeks

Guidelines recommend considering NPWT for postoperative diabetic foot wounds and for chronic DFUs that fail to improve after 4–8 weeks of standard therapy; require optimization of vascular status, infection control, and offloading before adjunctive NPWT

Clinical application: Use NPWT as an adjunct to best standard care (debridement, infection control, offloading, compression where appropriate) for chronic and neuropathic ulcers, and consider prior conservative therapy per guideline-recommended intervals before escalation

Open surgical wounds and grafts

NPWT has evidence of benefit for some open surgical wounds and in improving skin graft take in certain settings.

Open abdominal/laparotomy wounds: A recent RCT demonstrated reduced surgical site infections and other surgical site occurrences after laparotomy in high-risk patients treated with NPWT compared with conventional dressings

Consider NPWT in patients with risk factors for surgical site occurrences; note single-center and other trial limitations

Open abdomen/temporary abdominal closure: Systematic reviews report uncertainty regarding primary fascial closure and other outcomes for NPWT in open abdomen; evidence is limited by few eligible trials

Interpret in context of individual patient and surgical goals

Split-thickness skin grafts (STSG): Systematic reviews and clinical series report increased graft take and improved outcomes with NPWT compared with traditional bolster dressings, especially for irregular or contoured recipient beds

Consider NPWT to improve graft adherence and reduce seroma/hematoma when indicated

Open fractures/trauma wounds

Evidence for NPWT in open fractures is mixed: some meta-analyses suggest lower infection rates while large RCTs show no consistent benefit in patient-reported outcomes or infection.

Gustilo type III open fractures: Meta-analyses suggest lower overall, superficial, and deep infection rates with NPWT compared with conventional wound dressings in Gustilo 3 lower limb fractures

Evidence limited by heterogeneity and paucity of high-quality RCTs; use cautiously

Complex fractures with internal fixation: Large multicenter trials (e.g., WHIST) and other RCTs including WOLFF and WOLFF-like studies found no difference in long-term patient-reported disability or complication rates between NPWT and standard dressings

High-quality evidence does not consistently demonstrate patient-centered benefit for routine NPWT in these populations

Fracture-related infection (FRI): Evidence is insufficient to support NPWT as definitive therapy for fracture-related infection; NPWT may be used as temporary coverage pending definitive soft-tissue reconstruction

Document intent for temporary use and plan for definitive coverage when used in this context

Closed surgical incisions / prophylactic NPWT (ciNPT)

Evidence is heterogeneous: meta-analyses show possible reductions in surgical site infections and some surgical site complications in select populations, but many studies have risk of bias and some large RCTs show no benefit.

Evidence synthesis: Systematic reviews and updated Cochrane analyses report probable reduction in surgical site infections with prophylactic/closed-incision NPWT but inconsistent effects on wound dehiscence and other secondary outcomes; many studies have unclear or high risk of bias

Decisions should consider surgical type, patient risk factors, and heterogeneity of the evidence; routine use not universally supported

High-risk surgical cohorts: Meta-analyses and specialty-specific reviews suggest ciNPT may reduce surgical site complications, LOS, and readmissions in some high-risk populations (e.g., certain abdominal surgeries, obese cesarean patients), but large RCTs have reported neutral results for some indications

Consider ciNPT selectively for patients with specific risk factors; require clinical justification and documentation

Insufficient for routine prophylaxis: Because of inconsistent trial results, risk of bias, and heterogeneous patient populations, routine prophylactic application of NPWT to all closed incisions is not supported without specific patient risk factors or documented rationale

Pilonidal disease

Current evidence is insufficient to support routine NPWT for pilonidal disease.

Pilonidal post-operative use: Randomized and systematic review evidence is very low quality and does not demonstrate that NPWT reduces surgical wound dehiscence or improves healing times after pilonidal excision with primary closure

NPWT is not recommended routinely to prevent wound dehiscence in pilonidal disease

Pilonidal disease - coverage stance: There is insufficient clinical evidence demonstrating safety and/or efficacy of NPWT (including disposable systems) for treating pilonidal disease; NPWT is not recommended to prevent surgical wound dehiscence after pilonidal excision based on current RCT and systematic review findings.

Ensor RCT showed no difference in dehiscence or healing times; reviews rated evidence very-low quality.

NPWTi-d - promising but low-quality evidence

NPWT with instillation (NPWTi-d)

NPWTi-d - evidence summary: Systematic reviews and meta-analyses report higher primary wound closure rates and lower complication rates with NPWTi-d versus NPWT or standard care in heterogeneous wound populations, but the overall quality and heterogeneity of studies is limited and large randomized trials are needed

De Pellegrin et al. (13 studies, n=871) and Gabriel et al. reported benefits; pooled healed proportion estimates are high but evidence is low quality

Operational documentation: When NPWTi-d is requested, document wound type, prior debridements, concurrent therapies (e.g., antibiotics), number of debridements, and readiness for final closure to support clinical rationale

Prior authorization should reflect limited and heterogeneous evidence base

Diabetic foot - adjunctive NPWT: Guidelines suggest considering NPWT as an adjunct to best standard care for postoperative diabetic foot wounds and for chronic diabetic foot ulcers that fail to improve after 4–8 weeks of standard therapy; recommendation strength varies by context4-8 weeks

Ensure vascular status, infection control, and offloading are optimized prior to adjunctive NPWT

Pressure injuries: NPIAP and WHS recommend considering NPWT as an early adjunct for Stage III/IV pressure injuries to reduce size and depth and promote granulation

Consider NPWT when conventional therapy fails or to prepare for surgical closure

Routine use of negative pressure wound therapy (NPWT) is not supported for all closed surgical incisions or for pilonidal disease in the absence of specific, documented patient risk factors. The policy concludes there is insufficient clinical evidence demonstrating safety or efficacy of NPWT systems (including disposable/single-use devices) for treating closed incisions or pilonidal wounds, and available studies are too small or at risk of bias to support routine application.

Use of NPWT for pilonidal disease is effectively excluded from routine recommended use because current randomized trials and systematic reviews provide insufficient or very-low quality evidence of benefit. A recent RCT (Ensor et al., 2024) found no reduction in surgical wound dehiscence, no improvement in time to healing, and no clear patient-centered benefit with NPWT versus conventional dressings, and Hayes reviews characterize the overall evidence as very low quality.

The policy revisions updated contraindications for NPWT to explicitly include anticoagulation therapy and to clarify infection-related exclusions as uncontrolled soft tissue infection or untreated osteomyelitis within the vicinity of the wound. These changes are intended to reflect safety concerns when using subatmospheric pressure in patients at increased bleeding risk or with active, untreated contiguous infections.

For indications where the literature is limited, heterogeneous, or does not show clear patient benefit (for example, routine prophylactic NPWT on all closed surgical incisions and NPWT for pilonidal disease), the evidence is insufficient to support coverage absent compelling clinical justification. Providers should document the clinical rationale and relevant risk factors when requesting NPWT for these evidence-limited indications because such requests are at higher risk for denial.

In summary, NPWT for pilonidal disease is not supported by the current evidence base. The available RCT data do not demonstrate reductions in dehiscence or faster healing times, and systematic reviews rate the overall evidence as very low quality; therefore NPWT should not be used routinely to prevent surgical wound dehiscence after pilonidal excision.
