Covered in outpatient setting or upon discharge when ALL of the following are met for specified indications:

Complete Wound Therapy: Documentation that a complete wound therapy program was tried or considered and ruled out, including evaluation, care, wound measurements; dressings to maintain a moist wound environment; debridement of necrotic tissue if present; evaluation and provision for adequate nutrition; provider documentation of indication for NPWT; and documentation that the open wound has not responded to conventional treatment after 30 days.30 days

Pressure Ulcers

Evidence and recommendations for NPWT in pressure injuries/stage 3-4 pressure ulcers.

pressure_ulcer_evidence: RCT (Sahin et al. 2021) demonstrated greater granulation tissue formation, wound shrinkage, and decreased PUSH scores with NPWT versus wet-to-dry dressing in patients with stage III–IV pressure injuries, though the trial was small with short follow-up. Guideline recommendations (Vig et al. 2011) support use of NPWT to reduce wound dimensions, improve wound bed quality, and to use until surgical closure is possible or to achieve secondary intention closure.

Evidence supports NPWT as an adjunct to complete wound therapy for Stage III–IV pressure injuries.

Neuropathic / Diabetic Foot Ulcers (DFUs)

Multiple RCTs, systematic reviews, and meta-analyses assessing NPWT versus moist dressings or standard care for DFUs.

dfu_benefit: Systematic reviews/meta-analyses and RCTs (Wang 2022; Liu 2017; Zhang 2014; Wu et al. 2023; Hayes reviews) report NPWT is associated with higher healing rates, greater reduction in ulcer area, increased granulation tissue, shorter time to readiness for grafting, and lower risk of amputation compared with moist wound care or standard dressings; limitations include small trials, heterogeneity, and low-to-moderate certainty of evidence.

Reserve NPWT as adjunctive therapy and for wound-bed preparation per guideline recommendations; document comprehensive diabetic management and pressure offloading.

Venous Insufficiency Ulcers

Evidence limited but suggests potential benefit when first-line therapy fails.

Open Surgical Wounds and Skin Grafts

Mixed evidence for NPWT in open surgical wounds, temporary abdominal closure, and skin grafts.

open_abdomen_and_grafts: RCT evidence (Moreno et al.) demonstrated reduced surgical site infections and surgical site occurrences after laparotomy with NPWT; Cochrane and other reviews report uncertainty for open abdomen. Systematic reviews for split-thickness skin grafts show improved graft take and reduced complications with NPWT. Consider NPWT for graft retention or in patients with risk factors for surgical site complications, balancing benefits against uncertainty in some open-abdomen settings.

Open Fractures and Trauma-related Wounds

Evidence is mixed; some meta-analyses show reduced infections, while large RCTs sometimes show no difference in long-term outcomes.

Closed Surgical Incisions (Prophylactic / Incisional NPWT)

Evidence for prophylactic/incisional NPWT on closed incisions is heterogeneous and of variable quality; some meta-analyses report reduced surgical site complications while Cochrane updates indicate mixed certainty.

Pilonidal Disease

Current evidence does not support routine NPWT for pilonidal disease.

Pilonidal disease — coverage stance: There is insufficient clinical evidence demonstrating the safety and/or efficacy of NPWT systems, including disposable systems, for treating pilonidal disease; NPWT is not recommended to prevent surgical wound dehiscence after pilonidal surgery.

Based on RCT (Ensor et al. 2024) and Hayes report conclusions; further high-quality studies are needed.

NPWTi-d — promising but unproven

NPWT with instillation (NPWTi-d)

NPWTi-d — evidence summary: Systematic reviews and meta-analyses (De Pellegrin 2023; Gabriel 2021; Kanapathy 2020) report higher primary wound closure rates, reduced complications, fewer debridements, and improved bioburden reduction with NPWTi-d compared with NPWT or standard care across heterogeneous wound types; however, study quality is limited and pooled data derive largely from small or nonrandomized studies.

NPWTi-d is promising but unproven; large prospective RCTs are needed and documentation should include concurrent standard-of-care measures when used.

Use of negative pressure wound therapy (NPWT) for any indications not explicitly listed as covered in this policy is considered unproven and not medically necessary. This includes, but is not limited to, closed surgical incisions, pilonidal disease, disposable/single-use NPWT systems, and NPWT systems that perform instillation of wound solutions. Claims for outpatient NPWT for these non‑specified indications will be evaluated against the policy’s stated coverage criteria and supporting evidence; absent high‑quality evidence demonstrating safety and efficacy, such uses are not covered.

Routine or widespread use of NPWT for closed surgical incisions, including single‑use or disposable systems, is not supported by sufficient high‑quality evidence. Current studies are heterogeneous, often small or at risk of bias, and do not uniformly identify the patient populations most likely to benefit. Use of prophylactic or routine incisional NPWT should therefore be considered experimental/insufficient unless specific high‑risk features are documented and justification is provided.

NPWT is not recommended for the treatment of pilonidal disease based on the available evidence. Randomized trial data and systematic reviews to date do not demonstrate clear safety or efficacy for NPWT in this indication; therefore NPWT for pilonidal disease is explicitly considered not medically necessary.

Application of this policy must be consistent with the member specific benefit plan and any applicable federal or state mandates. When the member specific benefit plan differs from the standard policy, the terms of the member specific benefit plan govern. Providers should verify benefit and prior authorization requirements prior to initiating NPWT.

NPWT for the treatment of indications not expressly listed as covered—such as closed surgical incisions and pilonidal disease—as well as the routine use of disposable/single‑use NPWT systems and NPWT with instillation of wound solutions, are considered unproven and not medically necessary due to insufficient evidence of efficacy. Documentation that a use falls outside covered indications should be provided when seeking coverage or prior authorization.

Prophylactic or incisional NPWT for all closed surgical wounds and NPWT for pilonidal disease are not established as standard therapy. The evidence is inconsistent, low to very‑low certainty, or subject to bias, and therefore routine use without documentation of specific high‑risk surgical factors or other justification is not supported.

Use of NPWT after pilonidal surgery with the intent to prevent surgical wound dehiscence is not supported by current evidence and is therefore considered not medically necessary. Available randomized data showed no benefit in reducing dehiscence or improving healing time compared with conventional dressings.

A complete wound therapy program must be tried, or considered and ruled out, prior to initiation of NPWT. The provider must document: evaluation and care provided; wound measurements; application of dressings to maintain a moist wound environment; debridement of necrotic tissue if present; evaluation of nutritional status and plan; specific indication for NPWT; and that the open wound has not responded to conventional treatment after 30 days. Failure to document these elements may result in denial or retrospective review.

Step Therapy

Compression Therapy Required First

For venous leg ulcers, first-line therapy is compression. NPWT may be considered only if compression therapy was attempted and found not efficacious; document the trial of compression therapy and rationale for escalation to NPWT.

Step Therapy

Stepwise Approach for Diabetic Foot Ulcers

For diabetic foot ulcers, standard wound care and optimization (vascular status, infection control, offloading) should be used first. Reserve NPWT as an adjunctive therapy for wounds that fail to demonstrate expected healing (>50% area reduction) after an appropriate period (typically 4–8 weeks) of standard therapy; document wound progression, optimization steps taken, and rationale for NPWT.

Document re-evaluation of vascular status, infection control, and offloading prior to NPWT.

Denial Risk

Pilonidal Disease — Lack of Support

Use of NPWT for pilonidal disease is not supported by sufficient evidence. Providers should not routinely use NPWT for pilonidal disease; if considered, document clinical rationale and be aware claims may be at higher risk for denial based on lack of supporting evidence.

When used prophylactically (e.g., after abdominal surgery or other closed incisions), document patient risk factors that justify prophylactic NPWT and provide clear clinical rationale.
