Covered when ALL of the following are met per InterQual Medicare criteria
Policy defers to InterQual criteria for specific clinical requirements; when InterQual indicates therapy is medically necessary, HCPCS code E0769 is considered medically necessary for the covered service.
The U.S. Food and Drug Administration has not approved any electromagnetic devices specifically for the treatment of chronic wounds; therefore use of electromagnetic devices for wound healing is considered an off-label indication. FDA status is provided for informational purposes and does not by itself dictate coverage determinations.
Coverage for electromagnetic therapy is limited to situations that meet the external InterQual Medicare criteria: Procedures, Electrical Stimulation (ES) and Electromagnetic Therapy for the Treatment of Wounds.
