Coverage Criteria for Electrical Stimulation (ES)

inv-01: Covered Indication (Pressure Ulcers)

Covered when ALL of the following are met:

Coverage criteria for ES on pressure ulcers: 1) Pressure ulcer is Stage III or Stage IV; 2) The ulcer has failed to demonstrate measurable signs of healing after 30 days of conventional treatment; 3) Conventional treatment during the 30 days included: appropriate turning and positioning; application of dressings to maintain a moist wound environment; debridement of necrotic tissue, if present; management of infection, if present; evaluation of and provision for adequate nutritional status; moisture and incontinence management; and use of a pressure-reducing support surface.30 days

Electrical stimulation is considered not medically necessary for other wound types due to insufficient evidence, including but not limited to:

Protocols and sufficient trial data for these indications are lacking; clinical trial data are insufficient to prove efficacy or define optimal parameters.

inv-03: Clinical evidence and guideline context

Evidence summary and guideline positions relevant to coverage considerations:

Evidence from systematic reviews and RCTs: Meta-analyses and systematic reviews report that electrical stimulation (ES) may improve ulcer area reduction and healing rates in some studies for diabetic-related ulcers, venous leg ulcers, and pressure ulcers; however, included trials are heterogeneous, often small, and at variable risk of bias. Pulsed monophasic/high-voltage pulsed currents are among the most commonly studied modalities.

Guideline recommendations: Guideline bodies differ in recommendations: IWGDF and NICE advise against offering ES for diabetic foot problems outside clinical trials (IWGDF: strong recommendation, low-quality evidence; NICE: do not offer ES outside research). Society for Vascular Surgery/American Venous Forum do not recommend ES for venous leg ulcers. The American College of Physicians gives a weak recommendation supporting ES as adjunctive therapy for pressure ulcers (moderate-quality evidence).

These divergent positions inform cautious, indication-specific coverage.

Research limitations affecting coverage:

Electrical stimulation is covered only for Stage III or Stage IV pressure ulcers that have failed to demonstrate measurable signs of healing after 30 days of documented conventional care. Electrical stimulation for wounds other than Stage III/IV pressure ulcers that meet these coverage criteria (for example, diabetic ulcers or venous stasis ulcers) is considered unproven and not medically necessary due to insufficient evidence of efficacy.

No electrical stimulation device is FDA-approved specifically for the treatment of chronic wounds; use for wound healing is an off‑label indication. The FDA regulates electrical stimulation devices as Class II devices and has cleared more than 500 devices via the 510(k) process, but clearance is not approval for a chronic wound indication. Guideline bodies reflect limited and mixed evidence: NICE recommends against offering ES for diabetic foot problems outside clinical trials, and other specialty guidelines advise against routine use for venous leg ulcers.

The evidence base for using electrical stimulation to treat diabetic foot ulcers and venous stasis ulcers is limited and heterogeneous. Protocols defining optimal parameters, delivery strategy, and duration are not established, and available clinical trials are insufficient to demonstrate consistent benefit. Therefore, use of ES for these ulcer types is not supported and is considered not medically necessary outside of clinical research or unless future high‑quality evidence becomes available.

The International Working Group on the Diabetic Foot (IWGDF) concluded there is little published evidence to justify electrical therapies for diabetic foot ulcers and recommends not selecting interventions that alter the physical environment (including electricity) in preference to accepted standards of good quality care (GRADE: strong recommendation; quality of evidence: low). IWGDF notes that studies are few and often of poor methodological quality, limiting confidence in ES for diabetic foot management.

Background

Electrical stimulation (ES) delivers electrical current via electrodes placed on or around a wound to modify wound‑bed charge and cell membrane potentials, with the proposed physiological effects of promoting angiogenesis and enhancing cellular activity to accelerate healing. Devices employ a variety of waveforms and delivery approaches and are regulated by the FDA as Class II devices; however, no device is specifically approved for chronic wound treatment and use for wound healing is typically off‑label.

Electrical Stimulation as Adjunct to Standard Wound Care

Electrical stimulation may be considered only as an adjunct to standard wound care after documentation that standard care was provided. Clinical guidelines vary: some (ACP) support ES as an adjunct to standard care for certain pressure ulcers, while others (IWGDF, NICE, Society for Vascular Surgery/American Venous Forum) recommend against ES for diabetic foot ulcers and venous leg ulcers or only in the context of clinical trials. Coverage is limited to indications supported by the policy and documented evidence.
