- Appropriate turning and positioning.
- Application of dressings to maintain a moist wound environment.

- Evaluation of and provision for adequate nutritional status.
- Moisture and incontinence management.
- Use of a pressure-reducing support surface.
Electrical stimulation is used as an adjunct to, not a replacement for, standard wound care practices.
Treatment is ordered and documented by a qualified provider, with documentation of wound measurements and response to prior therapies.

Pressure Ulcers

Pressure ulcers — covered when ES is used as adjunctive therapy for recalcitrant or stage II–IV pressure injuries and supported by guideline evidence:

Diabetic Foot Ulcers and Venous Leg Ulcers

Diabetic foot ulcers and venous leg ulcers — not routinely covered due to inconsistent, low-quality, and heterogeneous evidence:

Major professional guidance discourages routine use: IWGDF recommends against selecting agents that alter the physical environment (including electrical therapies) over accepted standards of care (strong recommendation, low-quality evidence); NICE recommends ES only in the context of clinical research for diabetic foot problems; Society for Vascular Surgery/American Venous Forum does not recommend ES for venous leg ulcers.
Therefore, ES for diabetic foot ulcers (DFUs) and venous leg ulcers (VLUs) is considered to have insufficient evidence of efficacy and is not medically necessary except within clinical trials or where specific payer-approved exceptions apply.
