Background, Evidence Summary & Definitions

Nerve grafting (for example, sural nerve grafting) has been proposed to restore erectile function after radical prostatectomy when neurovascular bundles are resected; the sural nerve is commonly used as an expendable donor nerve harvested from the leg and grafted to the resected cavernous nerve.

Overall, the clinical evidence is limited and of insufficient quality to establish efficacy: available data consist primarily of single-arm case series and retrospective studies suggesting possible benefit but with substantial risk of bias, and one randomized trial that was terminated early after an interim analysis showed no increased potency with unilateral sural nerve grafting compared with control. Systematic review authors and guideline statements conclude that evidence is inadequate to support routine use and that further randomized trials with long-term follow-up are needed.
