North Carolina–only UnitedHealthcare community plan medical policy that defines medical necessity criteria for sacral nerve stimulation (SNS) screening trial, permanent implantation, replacement/revision for urinary voiding dysfunction and fecal incontinence in adults (≥18 years). Also states noncoverage for constipation and chronic pelvic pain due to insufficient evidence.
