Covered when ALL of the following are met

UnitedHealthcare will reimburse out-of-pocket expenses for services received while confined/incarcerated when the following conditions apply:

ALL of the following

Services were provided or authorized by the member's PCP or network medical group in agreement with the terms of the health plan
Services were emergency health care services or urgently needed services
Either emergency/urgent services qualify regardless of PCP authorization (per Covered Benefits text and related Emergency and Urgent Services policy).
Member is confined, incarcerated, detained, or, if a juvenile, detained in any facility
Refer to the member's Evidence of Coverage (EOC)/Schedule of Benefits (SOB) to determine eligibility
If discrepancy between this policy and EOC/SOB, EOC/SOB governs.

Not Covered

The following services are excluded from coverage:

ANY of the following

Services while confined, except as stated in the Covered Benefits section
Services required for injuries or illnesses experienced while under arrest, detained, imprisoned, incarcerated, or confined according to federal, state, or local law

What Providers Must Do

Documentation Required

Verify EOC/SOB and authorization

Verify the member's Evidence of Coverage (EOC)/Schedule of Benefits (SOB) and confirm that services provided while the member was confined were either provided or authorized by the member's PCP or network medical group per the plan terms before claiming reimbursement.

Follow the member's EOC/SOB to determine eligibility and plan-specific provisions.
Reimbursement applies only when services were provided or authorized by the PCP/network medical group or were emergency/urgent services.

Prior Authorization

Authorization requirement for non-emergent services

Obtain and document authorization from the member's PCP or the network medical group for any non-emergency services provided while the member was confined; such services must be provided or authorized per plan terms to be eligible for reimbursement.

Non-emergency services provided while confined require PCP/network medical group authorization in agreement with the terms of the health plan.
If there is a discrepancy between this policy and the member's EOC/SOB, the EOC/SOB governs.

Denial Risk

Denial risk for services related to arrest/detention injuries

Do not submit claims for services required for injuries or illnesses that occurred while the member was under arrest, detained, imprisoned, incarcerated, or confined, as these services are excluded and are at high risk for denial.

Services required for injuries or illnesses experienced while under arrest, detained, imprisoned, incarcerated, or confined are listed in the Not Covered section and are excluded from coverage.

Scope and Rationale

UnitedHealthcare will reimburse a member's out-of-pocket expenses for services received while the member is confined, incarcerated, or, if a juvenile, detained in any facility when those services were provided or authorized by the member's PCP or network medical group in agreement with the terms of the health plan, or when the services are emergency health care services or urgently needed services. Providers should refer to the member's Evidence of Coverage (EOC)/Schedule of Benefits (SOB) to determine plan-specific eligibility and limitations.

Liability limitations apply for services provided in state hospitals: UnitedHealthcare's liability for covered health care services provided in a state hospital is limited to the rate UnitedHealthcare would pay for those services if provided by a network hospital.

Services that are excluded from coverage include services while confined except as stated in the Covered Benefits section and services required for injuries or illnesses experienced while under arrest, detained, imprisoned, incarcerated, or confined according to federal, state, or local law.
