The policy conclusion reflects insufficient evidence of safety and/or efficacy to support these procedures as standard covered treatments.

Document imaging that demonstrates lymphatic anatomy and technical feasibility when proposing microsurgical or lymphatic procedures. Near-infrared fluorescence imaging (e.g., ICG) or other diagnostic imaging used to identify viable lymphatic vessels should be included in the record when cited as part of patient selection.
