Diagnostic performance summary: NAF cytology demonstrates limited sensitivity with high specificity in some series; a meta-analysis reported pooled sensitivity ~0.64 and specificity ~0.97 but noted a mean 38.9% sample inadequacy rate. Small studies (e.g., Sauter et al.) found very low sensitivities (10%–14%) in some cohorts, with combined approaches (NAF + MD and image analysis) improving sensitivity up to ~55% while retaining high specificity.

Jiwa et al. meta-analysis and Sauter et al. provide primary diagnostic accuracy data and note limitations of sample adequacy and ductal sampling coverage.

FDS diagnostic accuracy and limitations: FDS (ductoscopy) shows variable sensitivity and specificity across studies.
