Available evidence for fiberoptic ductoscopy (FDS) is limited to preliminary cross‑sectional studies assessing technical success and diagnostic accuracy; the procedure is technically challenging, examines only a small portion of the ductal system, and often cannot access peripheral lesions. There is no high-quality evidence that ductoscopy improves breast cancer detection rates, leads to earlier stage at diagnosis, or reduces breast cancer mortality, and further studies are required to demonstrate outcome benefits.

Practice guidance does not support routine use of ductal lavage or nipple smear cytology. The National Comprehensive Cancer Network (NCCN, 2025) states that ductal lavage (DL) is not recommended, and nipple smear cytology is not routinely recommended for patients with nipple discharge but no palpable abnormality. These recommendations align with the policy position that DL and nipple smear cytology are not indicated for routine screening or diagnostic use.

The U.S. Food and Drug Administration has noted that nipple aspirate tests are not substitutes for mammograms (FDA consumer update, October 26, 2023). This policy echoes that guidance and does not endorse nipple aspirate testing as an alternative to standard mammographic screening.

Cytologic examination of epithelial cells obtained from breast ductal fluids — including samples collected by ductal lavage (DL), nipple aspiration (nipple aspirate fluid, NAF), or during intraductal visualization with fiberoptic ductoscopy (FDS) — has been investigated as a potential early indicator of breast cancer. Studies have reported low sensitivity, substantial sample inadequacy rates, and heterogeneous, largely preliminary evidence, and there is limited data demonstrating an impact on clinically meaningful outcomes such as stage at diagnosis or mortality. For these reasons, cytologic evaluation of ductal fluids is not supported as a standard screening or diagnostic modality.
