The following are considered unproven and not medically necessary for breast cancer screening, breast cancer diagnosis, or as alternative tools to guide surgery:

Insufficient evidence of efficacy to support use for screening, diagnosis, or surgical guidance.

Unproven procedures: Breast ductal fluid aspiration and cytology (nipple aspirate fluid cytology)

Unproven procedures: Fiberoptic ductoscopy (FDS), with or without ductal lavage

Evidence summary and considerations

Summary of evidence: Multiple single-center retrospective cohorts and small prospective studies report variable sensitivity and specificity for nipple aspirate fluid (NAF) cytology, ductal lavage (DL), and fiberoptic ductoscopy (FDS). Meta-analyses and systematic reviews show high pooled sensitivity for ductoscopy when any visualized finding is considered positive but lower specificity; NAF cytology has low sensitivity (pooled sensitivity ~0.64) and high specificity (~0.97) with a substantial proportion of inadequate samples (mean >=38.9%). Randomized data are limited and do not demonstrate differences in pathological yield or surgical specimen size.

Situations where FDS may add diagnostic value: FDS may detect intraductal lesions in patients with pathologic nipple discharge (PND) when ultrasound and mammography are negative; combining high-frequency ultrasound (HFUS) with FDS improved diagnostic accuracy in some series.

Supported by Zhang et al. (2025), Yuan et al. (2022), and related cohort studies (chunks 20,21,22).

Limitations and risks: DL and NAF cytology have low sensitivity and high rates of inadequate or inconclusive specimens; FDS is invasive, technically challenging, examines only a portion of the ductal system, may miss peripheral lesions, and has reported procedure failure and minor complication rates. Most evidence is single-center, retrospective, and heterogeneous, with limited prospective randomized trial data demonstrating clinical outcome benefits.

The policy identifies breast ductal lavage, breast ductal fluid aspiration and cytology (nipple aspirate fluid, NAF), and fiberoptic ductoscopy (FDS) as procedures that are unproven and not medically necessary for breast cancer screening, diagnosis, or as alternative tools to guide surgery due to insufficient evidence of efficacy.

For informational purposes the FDA notes that devices for collecting ductal fluid are listed under product code KNW, but FDA approval alone is not a basis for coverage. The FDA has also stated in consumer guidance that nipple aspirate tests are not substitutes for mammograms.

Summary: ductal lavage, nipple aspirate fluid cytology, and fiberoptic ductoscopy are considered not medically necessary for routine breast cancer screening, for establishing breast cancer diagnosis, or as alternative tools to guide surgical decision-making because available studies do not demonstrate reliable clinical benefit.

There is no high-quality evidence demonstrating that fiberoptic ductoscopy improves overall breast cancer detection rates, leads to earlier stage at diagnosis, or reduces breast cancer mortality; published studies are mainly preliminary, single-center, or cohort designs with heterogeneity and limited prospective randomized data.

Background: Cytologic analysis of epithelial cells obtained from breast ductal fluids—collected via ductal lavage, nipple aspiration, or fiberoptic ductoscopy—has been investigated as a potential early indicator of breast cancer. Studies report low sensitivity, high rates of inadequate or inconclusive specimens, and limited evidence that these techniques improve cancer detection rates or mortality compared with standard screening; therefore their routine clinical use is not supported.

The following uses of ductal lavage, ductal fluid aspiration/cytology, nipple smear, and fiberoptic ductoscopy are considered not medically necessary due to insufficient evidence and should not be performed for these indications:

Screening for breast cancer.
Routine diagnosis when standard diagnostic pathways (mammography, clinical breast exam, guideline-directed imaging) suffice.
As an alternative to or substitute for guideline-recommended imaging to guide surgical planning.

Documentation Required

Documentation and FDA Guidance

Document the clinical rationale for diagnostic testing and include prior diagnostic imaging and findings in the medical record. When billing unlisted procedure codes (e.g., CPT 19499), include thorough documentation to support medical necessity and coding rationale. Note that FDA information indicates nipple aspirate/NAF tests are not substitutes for mammography.

Include results of mammography, ultrasound, MRI, and clinical breast exam in documentation.
For CPT 19499, supply detailed operative or procedure report, indication, and why standard methods were inadequate.

Preferred Standard Evaluation and Use of Noninvasive Methods

Preferred standard evaluation for pathologic nipple discharge (PND) and other breast symptoms follows guideline-directed screening and diagnostic pathways. Noninvasive imaging such as diagnostic mammography and high-frequency ultrasound (HFUS) should be used as initial evaluations when appropriate; invasive intraductal procedures are generally reserved for cases where noninvasive methods are inconclusive and clinical management would be altered by additional findings.

Follow standard screening and diagnostic pathways: clinical breast exam, diagnostic mammography, and guideline-directed imaging.
Consider HFUS as the preferred initial evaluation for PND when appropriate; reserve invasive ductoscopic procedures for cases not clarified by noninvasive imaging.
If noninvasive imaging establishes lesion cause, location, and extent, additional intraductal procedures are typically unnecessary.

DefinitionEndoscopic visualization of breast ducts using a very thin endoscope, enabling direct inspection and targeted sampling of intraductal lesions.

LimitationsTechnically challenging, can be uncomfortable, examines only a small portion of the ductal system, and peripheral lesions may be inaccessible.

Alternate definitionNAF may be collected noninvasively or via fine-needle aspiration and used for cytologic evaluation and image analysis.

Combination useIn studies, NAF has been combined with mammary ductoscopy (MD) cytology and image analysis; combining NAF + MD cytology and aneuploid image analysis improved sensitivity while maintaining high specificity (Sauter et al.).

Clinical limitationSensitivity of NAF cytology alone has been low in some cohorts, and performance varies with presence of pathologic nipple discharge (PND).
