Coverage Criteria for Sleep Studies

Specialized testing coverage criteria

Specialized testing may be considered medically necessary in select clinical situations. Coverage is defined below for actigraphy and for multiple-sleep-latency testing (MSLT) / maintenance of wakefulness testing (MWT).

ALL of the following

Actigraphy is covered only when used as an adjunct to other diagnostic testing or clinical assessment and NOT as a stand-alone diagnostic test.
Indications for actigraphy coverage include: monitoring total sleep time prior to Multiple Sleep Latency Test (MSLT) or for assessment of circadian rhythm sleep-wake disorders when used in conjunction with clinical evaluation and sleep logs.
Actigraphy is NOT covered for the routine diagnosis of obstructive sleep apnea, for replacement of polysomnography or HSAT, or for diagnosis of periodic limb movement disorder (EMG required).
Consistent with AASM guidance that actigraphy should not replace PSG or EMG for diagnosis; evidence does not support stand-alone diagnostic use.
Documentation must include rationale for use, concurrent sleep logs or HSAT/PSG results when applicable, and how actigraphy findings will influence management.

ALL of the following

MSLT and MWT are considered medically necessary to evaluate excessive daytime sleepiness and to support diagnosis of central disorders of hypersomnolence when performed following AASM procedural standards.
Prior to MSLT/MWT, objective evidence of sufficient nocturnal sleep should be documented (sleep diary, actigraphy, or PSG) to confirm adequate sleep opportunity and to interpret daytime testing results appropriately.
MSLT/MWT are NOT indicated as diagnostic tools for obstructive sleep apnea; they are intended to assess daytime sleep propensity and to aid in diagnosing narcolepsy or other central hypersomnolence disorders.
Documentation must include prior sleep evaluation results, medication and substance use history that could affect testing, and sleep opportunity assurance.

A comprehensive sleep evaluation must be documented in the medical record prior to diagnostic testing. Documentation should include relevant history (sleep symptoms, daytime sleepiness, comorbid conditions, medication use including opioids, prior testing and results), physical exam findings (airway assessment, BMI), and the clinical rationale for the chosen test type (HSAT vs PSG). For repeat or follow-up testing, provide indication and rationale (e.g., persistent symptoms despite good PAP adherence, significant weight change, new or worsening cardiopulmonary disease, unexplained device-generated data).

Background and Rationale

Obstructive sleep apnea (OSA) is characterized by repetitive collapse of the upper airway during sleep, producing intermittent hypoxia, sleep fragmentation, and daytime symptoms such as excessive sleepiness. OSA is associated with important cardiometabolic and neurocognitive consequences that can affect quality of life and safety.

Diagnostic evaluation differentiates laboratory-based attended polysomnography (PSG), which provides multi-channel physiologic recordings including sleep staging, from home sleep apnea testing (HSAT), which records respiratory and oxygenation parameters without EEG-based sleep staging. A PAP titration may be performed in-lab to determine therapeutic pressures.

Because diagnostic thresholds and subsequent management depend on accurate detection of respiratory events, the policy emphasizes use of technically adequate testing (for example, HSAT with a minimum of 4 hours of recording and required sensors, or attended PSG when indicated) rather than relying on less-validated modalities such as actigraphy for primary diagnosis.

Attended polysomnography (PSG) and HSAT have different technical and documentation expectations. Attended facility-based PSG must include full-channel monitoring (EEG, EOG, EMG, airflow, respiratory effort, pulse oximetry, ECG/heart rate) per AASM standards and InterQual facility-based Sleep Studies criteria. HSAT devices must be technically adequate (minimum 4 hours of recording and sensors including nasal pressure, chest and abdominal respiratory inductance plethysmography, and oximetry) and the report must document technical adequacy and hours of valid recording. If HSAT is negative, inconclusive or technically inadequate, AASM recommends performing attended PSG.

Testing Pathway Preferences

Follow established testing pathway preferences: for uncomplicated adult patients with signs and symptoms indicating increased risk of moderate to severe OSA, HSAT with a technically adequate device is generally preferred. Use attended PSG for patients with significant cardiorespiratory disease, suspected hypoventilation, neuromuscular weakness, chronic opioid use, prior stroke, severe insomnia, pediatric patients, or other complex/atypical presentations. When initial HSAT or PSG is negative but clinical suspicion remains, consider repeat testing with PSG per AASM guidance.

PAP Initiation Pathway

When initiating positive airway pressure (PAP) therapy, follow the InterQual CP: Procedures, Sleep Studies sequencing for initiation and titration pathways. Decisions about split-night studies, in-lab attended PAP titration, or home auto-PAP/APAP initiation should align with InterQual criteria and be supported by the documented clinical evaluation. For HSAT-diagnosed patients considered for APAP/PAP initiation, ensure appropriate follow-up and objective adherence and efficacy data are available; if device data are unexplained or symptoms persist, consider attended PSG.
