Coverage Criteria for Sleep Studies and PAP Titration

Home Sleep Apnea Testing (HSAT) - Adults

Covered when ALL of the following are met:

HSAT indication: Adult with suspected obstructive sleep apneaadult

APAP may be used to determine fixed PAP pressure where HSAT indicated

Attended full-channel Polysomnography (PSG) - Required conditions

Attended PSG in a healthcare facility is medically necessary when ANY of the following are present:

PSG required: Results of previous HSAT are negative, indeterminate, or technically inadequate to make a diagnosis of OSA

PSG required: Individual is a child or adolescent (age < 18 years)age < 18

PSG required: Individual has one or more comorbid medical conditions that prohibit use of HSAT

Comorbidities include: FEV1% predicted < 60; progressive neuromuscular disease; NYHA class III/IV heart failure or LVEF ≤40; BMI > 50; Obesity Hypoventilation Syndrome; documented ongoing epileptic seizures with sleep disorder symptoms; chronic opiate medication use > 3 months

Attended PSG - Other indications

Attended PSG is medically necessary when symptoms suggest the following conditions:

Other indications: Periodic Limb Movement Disorder (PLMD) not due to another disorder (e.g., not leg movements associated with sleep-disordered breathing)

Other indications: Restless Legs Syndrome (RLS)/Willis-Ekbom Disease that has not responded to treatment

Other indications: Parasomnia with documented disruptive, violent, or potentially injurious sleep behavior suspicious for REM Sleep Behavior Disorder (RBD)

Other indications: Narcolepsy once other causes of excessive sleepiness have been excluded by appropriate clinical assessment

MWT medically necessary: Adult who is unable to stay awake resulting in a safety issue OR assessing response to treatment in adults with sleep disordersadult

Attended PAP Titration

PAP titration: Split-night PSG for combined diagnosis and PAP titration when criteria for attended PSG are met; or full-night attended PSG for PAP titration when split-night is inadequate or not feasible and OSA is confirmed

Repeat PSG: Repeat attended full-channel PSG and repeat PAP titration for individuals with persistent or new symptoms despite documented appropriate current treatment or PAP therapy after addressing equipment/interface issues and medical problems

Considerations for multiple-night testing

Attended, full‑channel polysomnography (PSG) performed in a laboratory is considered not medically necessary for the evaluation of circadian rhythm disorders, depression, or insomnia because current evidence does not demonstrate efficacy for these indications. Actigraphy likewise has limited evidence and is not supported as a stand‑alone diagnostic substitute for PSG in these disorders; it may provide adjunctive objective sleep/wake information but is insufficient alone to establish diagnosis or change management in most cases.

When a single unattended home sleep apnea test (HSAT) is negative, inconclusive, or technically inadequate, performing another routine HSAT is discouraged because the likelihood of a second non‑diagnostic test is high. In these situations the policy strongly recommends proceeding to an attended, in‑lab PSG to obtain a definitive diagnostic evaluation rather than repeating HSAT.

The Maintenance of Wakefulness Test (MWT) is not medically necessary for children and adolescents <18 years. Abbreviated daytime sleep studies (for example, PAP‑Nap) and actigraphy lack sufficient evidence to support routine use as diagnostic alternatives, and are considered not medically necessary or investigational for many indications pending stronger data.

Actigraphy provides objective estimates of sleep/wake patterns and can be useful as an adjunct (for example, to estimate total sleep time before daytime testing or to support evaluation of circadian rhythm disorders), but current systematic reviews conclude that the evidence is limited and heterogeneous. Therefore actigraphy is not established as a stand‑alone diagnostic test for most sleep disorders and its use as the sole basis for diagnosis is not supported by the evidence.

The Multiple Sleep Latency Test (MSLT) is indicated for evaluation of suspected narcolepsy and to help differentiate idiopathic hypersomnia from narcolepsy, but it is not routinely indicated in the initial evaluation or diagnosis of obstructive sleep apnea (OSA), nor for routine assessment of change following OSA treatment such as CPAP. If excessive sleepiness persists despite optimal OSA therapy, directed evaluation including MSLT may be appropriate.

Prior Authorization and Documentation Expectations

Prior authorization may be required by the member's plan. When prior authorization is requested, submit complete medical records and documentation that support medical necessity for the specific sleep study or PAP titration. Incomplete documentation may delay review or result in denial.

Include relevant medical history and physical examination findings that support the clinical suspicion of a sleep disorder (e.g., daytime sleepiness, witnessed apneas, loud snoring, gasping/choking).

Background and Rationale

Obstructive Sleep Apnea (OSA) is a sleep‑related breathing disorder characterized by reduction or cessation of airflow despite continuing respiratory effort. Severity is graded by the Apnea–Hypopnea Index (AHI) or Respiratory Disturbance Index (RDI): mild = AHI/RDI ≥5 and <15 events/hour; moderate = ≥15 and ≤30 events/hour; and severe = >30 events/hour. PSG is the standard diagnostic test when there is concern for OSA, while HSAT may be used in uncomplicated adults at increased risk of moderate‑to‑severe OSA provided device and study quality criteria are met.
